Job summary

We are currently looking for a full time Health Care Assistant who will be working as a member of the St Ives District Nursing Community Team undertaking delegated duties in order to meet the needs of the needs of the service users.

You will be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users.

Main duties of the job

As a Community Health care Assistant you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. We are looking for caring, responsive and diverse people who share our passion to deliver our service.

Experience of working in the community. Experience of working within health or social care. Experience of caring for and supporting adults and older people Demonstrate a commitment to high quality care & service provision. Good communication skills. The ability to work flexibly and as part of a team, as well as individually.

Job responsibilities

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To work as a member of an interdisciplinary community healthcare team, working across health and social care boundaries. With training, use skills from the professional areas of; Nursing, Allied Health Professionals and Medicines Management to undertake tasks as delegated in order to meet the individual needs of service users. To assist in the delivery of high standards of health care and support patients within their community setting by following prescribed packages of care. Apply competency based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan. To deliver functional assessments, personal care, medication prompts, including mobility and appropriate exercise ,personal and domestic ADLs, equipment provision To provide a high standard of care

Job Responsibilities

Make judgements on patients response to prescribed treatment/care plans, and report findings to a qualified professional in line with competency framework. Overcome barriers to understanding, patients with hearing or cognitive impairment. Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals. Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately. Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner. Demonstrate an awareness and understanding of consent, and gain consent as appropriate following organisational policies. To attend relevant external and internal courses to extend knowledge or gain relevant skills to improve clinical practice as identified in the Personal Development Review (PDR). Will be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies. To attend meetings as required. Ensuring patients personal care is provided and highlighting areas of concerns such as pressure areas and reporting any changes to professional Work within the wider disciplinary team and recognise own role within it. Promote patient independence and self-management Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body's guidelines. To maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne or RIO within 24hours). Plan and prioritise own visits to delegated patients, (and ensure these are documented on SystmOne) Ensuring patients personal care is provided, as appropriate. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. To assist in the implementation of audit processes under direction from registered staff

You may be required to undertake and successfully complete the new national Care Certificate within a 12 weeks timeframe if you meet our criteria. You will receive full training and support and The Care Certificate sets out 15 national care standards that must be achieved by all health and social care staff to demonstrate that they have the skills and knowledge needed to provide safe and excellent quality of care. It is a foundation programme which can provide the first steps to career development within the NHS.
