Salutem PCN is a limited company formed on 1st January 2024 by5 GP Practices situated in the North-East of Leicester, to provide clinicalservices to our 52,000 patients. Our 5 GP Practices cover Downing DriveSurgery, East Leicester Medical Practice, Humberstone Medical Centre, JohnsonMedical Practice and St. Elizabeth Medical Centre. Our 5 practices are all highachieving and work collaboratively with Leicestershire community services tofocus on local health needs and population health to deliver contracts andservices for their GP registered population.

You will be working and based between Downing Drive Surgery and St. Elizabeth Medical Centre.

Main duties of the job

Job Summary

Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.

Support the medicines management team, in order to maximise cost-effective prescribing and improve the quality of patient care.

Under the direction of the Practices, we expect that you will be able to work independently and manage your own workload. You will need excellent communication skills to be able to liaise with patients, primary care staff, community pharmacists and other partners.

Downing Drive is a Practice based in Evington, Leicester and has a population of over 6,800 patients where all patients have an allocated named GP. They are a Training Practice and have GP Registrars working at the surgery.

About St. Elizabeth Medical Centre

St. Elizabeth is a Practice based in Netherhall, Leicester and has a population of over 7,063 patients where all patients have an allocated named GP.

Job responsibilities

Core Duties and Responsibilities(Key Result Areas)

-Work as part of a multi-disciplinary team in a patient facing roleto clinically assess and treat patients using their expert knowledge ofmedicines for specific disease areas.

-Be responsible for the care management of patients with chronicdiseases and undertake clinical medication reviews to proactively manage peoplewith complex polypharmacy, especially the elderly, people in care homes, thosewith multiple co-morbidities (in particular frailty, COPD and asthma) andpeople with learning disabilities or autism (through STOMP Stop OverMedication Programme);

-Provide specialist expertise in the use of medicines whilsthelping to address both the public health and social care needs of patients atthe PCN and to help in tackling inequalities.

-Provide leadership on person-centred medicines optimisation(including ensuring prescribers in the practice conserve antibiotics in linewith local antimicrobial stewardship guidance) and quality improvement, whilstcontributing to the quality and outcomes framework and enhanced services.

-Through structured medication reviews, support patients to taketheir medications to get the best from them, reduce waste and promoteself-care.

-Have a leadership role in supporting further integration ofgeneral practice with the wider healthcare teams (including community andhospital pharmacy) to help improve patient outcomes, ensure better access tohealthcare, and help manage general practice workload.

-Develop relationships and work closely with other pharmacyprofessionals across PCNs and the wider health and social care system.

-Take a central role in the clinical aspects of shared careprotocols, clinical research with medicines, liaison with specialistpharmacists (including mental health and reduction of inappropriateantipsychotic use in people with learning difficulties), liaison with communitypharmacists and anticoagulation; and

-Be part of a professional clinical network and have access toappropriate clinical supervision.

-High risk drug monitoring and get involved in audits, set by ICB/NHSEor towards borough wide aims and targets. Run searches for local and nationalmedication alerts/shortages.

-Review the on-going need for each medicine, a review of monitoringneeds and an opportunity to support patients with their medicines takingensuring they get the best use of their medicines (, medicinesoptimisation). Make appropriate recommendations to Senior Pharmacists or GPsfor medicine improvement.

-Support the pharmacist team to provide clinical medication reviewin care homes.

-Signposting to community pharmacy and referring to GPs or otherhealthcare professionals where appropriate.

-Do appropriate database searches, in keeping MHRA and NICE alerts/guidance to support safe patient care.

-Management of medicines at discharge from hospital.

-Set up and manage systems to ensure continuity of medicines supplyto high risk groups of patients.

-Undertake clinical audits of prescribing in areas directed by theGPs, feedback the results and implement changes in conjunction with thepractice team.

-Work with the general practice team to ensure the practice iscompliant with CQC standards where medicines are involved.

Person Specification

Qualifications

Essential

-Independent prescriber -Mandatory registration with General Pharmaceutical Council -Masters degree in pharmacy -Minimum of 2 years post qualification experience. -An appreciation of the nature of GPs and general practices. -An appreciation of the nature of primary care prescribing, concepts of rational Excellent interpersonal, influencing and negotiating skills. -Excellent written and verbal communication skills. -Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (, patients). -Can plan, manage, monitor, advise and review general medicine optimisation. -Self-Motivation. -Adaptable. -Ability to travel between the two surgeries. -In date CRB/DBS check. -Immunisation status.

Desirable

-Membership of the Royal Pharmaceutical Society. -Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience. -Enhanced Diabetes skills. -Safeguarding adult and children level three. -Information governance toolkit completion. -Basic life support training.
