Job summary

We are looking for an experienced,motivated and caring health care assistant to join our friendly Cityside NeighbourhoodTeam.

Your role within theteam is essential in allowing patients to achieve optimum function within theirown home.

The Neighbourhood Team operates from07:30-18:30 each day and you will work , 08:30-16:30 or 10:30-18:30shift patterns on a roster basis.

Our Neighbourhood team consists ofCommunity Nurses, Physiotherapists, Occupational Therapists, HealthcareAssistants and administrative staff; with GP and social care colleaguesworking closely alongside.

Main duties of the job

While on duty you will either be working alone or alongsidecolleagues to support delegated tasks in line with Trust standards and corevalues. Each task will have been assessed by a qualified Health careprofessional and a care plan will be in place for you to follow and report backon. Tasks may include personal care, supporting daily activities, participatingin manual handling and other allocated tasks such as dressing wounds, settingup equipment, and recording observations. All clinical documentation isrecorded in line with Trust guidelines and you will give daily handovers toteam leads summarising your involvement with your allocated patients. Clear andtimely Communication is key to your role which may be in a verbal, non-verbalor written format to support patient centred care.

Job responsibilities

Maintaining accurate and contemporaneous records inaccordance with Trust policies/procedures; while maintaining confidentiality atall times.

Ensuring a safe working environment by adhering to Trustpolicies and health and safety guidelines

Provide a high standard of outcome orientated care andsupport following a care/recovery plan under the supervision and guidance ofqualified health care professionals

Maintaining effective communication systems within theintegrated team, with other agencies, the voluntary sector, the client andcarer.

Qualifications

Level 3 qualification ( NVQ 3, A-level) in Care or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
