Main duties of the job

We are seeking an exceptional and organised ACP to join our incredible and dedicated Specialist Palliative care team at Rainbows Childrens Hospice, covering all 5 counties of the East Midlands: Hospice, Hospital and Community. In this vital role, you will provide advanced clinical expertise, contribute to symptom management , lead on end of life care and work closely with families to ensure the best quality of care for the babies, children and young people in our care. You will also support the Head of Palliative Care in managing, leading and introducing new services within the Hospice and manage a team of CNSs and unregistered professionals

Job responsibilities

You will be based at the Hospice in Loughborough and be required to travel across the East Midlands covering all 5 counties: Leicestershire, Nottinghamshire, Derbyshire, Northamptonshire and Lincolnshire. Hours of work: This role will be between the hours of 8:00am and 8:00pm, on a rota covering 7 days a week. There will be a requirement to be on the on-call rota. About the role Be responsible for the daily care of Babies, Children & Young People resident in the Hospice and those receiving end of life care within the hospitals and community across the East Midlands.Some of the key responsibilities include (but not limited to): To have lead nursing responsibility for a defined caseload of Babies, Children and Young People with Palliative and end of life care needs within the hospice, hospital and community.Be an independent prescriber & support the prescribing/transcribing processTo professionally support staff within the hospice, hospital and community to ensure that childs needs are assessed, care planned, implemented and evaluated, and that there is consultation and involvement of child/parents and carers. Liaison with staff and services within Hospital both Paediatrics and Neonates to identify and facilitate the transfer plan and subsequent ongoing care needs of Babies, Children & Young People.To be a member of the senior nurse team, demonstrating effective leadership skills and taking part in the on call rota.Further responsibilities in the role of an Advanced Clinical Practitioner at Rainbows, can be found by downloading the Job Description. What were looking for: Qualifications: Advanced Clinical Practitioner AND/OR MSc, Current registration with NMC/HCPC/GPC, Non-medical prescriber, History taking and clinical examination qualification.Experience: Significant Post Registration paediatric palliative care experience, Demonstrative track record working with own paediatric/neonatal client base, managing the assessment, treatment and discharge of patients and the ability to work in partnership with a range of other professionals including health, social care and education.Excellent Communication Skills: The ability to effectively engage both verbally and written with team members, nurses, and families in a sensitive and professional manner as well as manage challenging conversations effectively and with compassion.Effective organisational Skills: Strong ability to prioritise tasks and manage competing demands within a fast-paced care environment.Further requirements can be found by downloading the Person Specification. Significant Post Registration paediatric palliative care experience Demonstrative track record working with own paediatric/neonatal client base, managing the assessment, treatment and discharge of patients. Demonstrable ability to work in partnership with a range of other professionals including health, social care and education. Experience and evidence of working with Babies, Children and Young People with complex needs. Experience of managing safeguarding issues in clinical practice. Excellent communication skills, including the ability to communicate both verbally and in writing with a range of people in different contexts Experience of working with technology dependent children Effective organisational skills and ability to prioritise Awareness and understanding of multi-agency working Current knowledge and understanding of Safeguarding issues and procedures Current knowledge and understanding of developments in childrens and professional nursing and health care Understanding of the recent NHSE and NICE guidelines for childrens palliative care, and other relevant legal frameworks such as the Mental Capacity Act. Ability to demonstrate safe clinical practice including risk assessment Ability to manage change effectively Resilient and able to meet the complex demands of the post, able to take on unscheduled tasks and work competently and effectively under pressure Flexible approach with ability to adapt to changing or conflicting priorities Ability to work autonomously and with unpredictable work patterns, managing time, work commitments and priorities within the working hours allocated.
