An exciting opportunity has arisen to join the Acuteintervention team (AIT). The AIT service supports the provision of care acrossthe Trusts three acute sites by providing education and support for allhealthcare staff caring for the acutely ill deteriorating patient. Facilitatingdecision making and communication between clinical staff, patients andrelatives is a key component of the role. The Team is firmly committed toensure all patients are treated by the right professional, in the right place,at the right time, first time, every time, 24 hours-a-day.

This is an exciting role that will allow you toutilise your education skills and clinical skills to improve practice andensure that patients receive the best possible standard of care. The role wouldsuit a healthcare assistant with a background in acute/emergency/critical carewho has an interest in teaching.

Main duties of the job

Tosupport the delivery of the acute intervention team agenda to ensure care ofthe acutely ill patient is of a high standard.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description

Job responsibilities

To develop skills, knowledge and relationships which enableprofessional, timely and creative service delivery and development, based onpersonal development plans.

Follows standard procedures and guidelines and usesanalytical and judgement skills to assess patients who are acutely unwell.

Respond to emergency calls within the organisation as anactive member of the team.

Perform Task allocation within the hospital at night teamutilising the electronic systems.

Completes monthly audits within the Acute Intervention Teamand supports senior staff with audit.

Deliver teaching to the ward teams based on thedeteriorating patient and patient safety themes.

Person Specification

Qualifications

Essential

Relevant NVQ Level 3 or equivalent relevant experience. Care Certificate or willingness to complete within first 6 months of commencement in post. GCSE equivalent in English and Mathematics.

Desirable

Short courses relevant to undertake patient and clinical care duties.

Experience

Essential

Computer skills. Ability to learn new skills.

Desirable

Previous experience in caring for others.

Personal Attributes

Essential

Good interpersonal skills. Able to engage and build effective relationships at all levels. Understanding of the role of HCSW. Able to work as part of a team. Knowledge and understanding of the need for confidentiality.

Special Requirements

Essential

Self-motivated and able to motivate others. Excellent communication & interpersonal skills. Reflective practitioner. Ability to work in a busy and varied work environment and respond to change.

Desirable

Be able to offer pastoral care to mentors and students when providing feedback on performance.
