Barts Health NHS Trust invites applicants for a ST3 equivalent/Trust doctor post in Haematology, based at St Bartholomew’s Hospital (SBH). This post is to cover a short term gap in the NE London Haematology and Oncology training rotations. This post is available from 1st August 2024 and is for 6-12 months in the first instance. Full MRCP (or equivalent) is an essential requirement and your application will not be considered without it.

The post holder will work under the clinical direction of all the consultants in the respective Haematology or Haemato-Oncology Departments with a clearly identified clinical and educational supervisor. The post would be ideal for those wishing to pursue a career in Haematology and who subsequently wish to apply for a National Training Number.

As such the post is suitable for experienced SHO equivalents/Clinical Fellows/Registrars who plan to pursue a career in Haematology and who would like to add to their experience in Haemato-oncology. 

SBH is a busy and well supported level 3 Haemato-oncology unit an extensive Bone Marrow Transplant and CAR-T cell therapy programme. Clinical duties within the Haemato-oncology Department will cover the whole range of haematological malignancies, with the post-holder rotating between the inpatient and day-unit clinical areas while at SBH. 

Main duties of the job

At various times, they will have clinical responsibility for patients admitted to the wards, those ambulatory patients seen on the Day unit and those seen in outpatients. Support to become proficient in bone marrow biopsy, intrathecal chemotherapy and chemotherapy prescribing will be provided, as will clinical trial experience and opportunities to take part in quality improvement and research projects.

The post holder will be expected to prioritise and time-manage effectively according to the activity of the department and to utilise appropriately the available senior staff for advice. The Department has a key awareness of the importance of clinical audit, and all specialist trainees are encouraged to complete audit projects whilst here. Participation in undergraduate and CMT teaching is also encouraged.

This post does not have educational approval from the postgraduate dean and will not be recognised for training, however support will be provided for career development. In-house SpR training sessions are currently running and attendance at Regional training days is strongly supported

Detailed job description and main responsibilities

Share workload with other Ward SpRs in an equitable manner.

Assist the Day unit teams where possible for example clerking in ward emergency admissions on the Day unit.

Assist the other firm by offering to take on practical procedures if there is an imbalance in activity.

Day to day management of haematology inpatients on the Barts site

Ensure a daily review of all patients and their results (including results endorsement) in conjunction with SHO’s and liaison with the Attending consultant as indicated

Ensure all chemotherapy is prescribed and administered in a correct and timely fashion – liaise with unit pharmacist as necessary.

Provide daily clinical review of CAR-T cell therapy and transplant patients and their blood results including ciclosporin/tacrolimus levels and ensure the relevant medications are taken as required.

Write CAR-T / BMT protocols for patients having cellular therapy procedures. Ensure these are available for the BMT meeting for discussion 1 week prior to admission (even if information still outstanding).

Provide point of contact for Haematology referrals from other hospitals. This may include arranging ward admission if necessary. To discuss them with Attending consultant

Communicate closely with Attending consultant, non-attending consultants, ward sisters, CNS’s and Day Unit staff

Performing practical procedures as necessary and arrange staging investigation/MDM discussion

Discharge summaries, coding and communication with GP’s/referring consultants

MBBS or equivalent Full MRCP(UK) Evidence of achievement of Foundation (FY1/2) competencies or equivalent Evidence of achievement of CMT1/2 competencies or equivalent

Desirable criteria

Intercalated BSc or equivalent Higher degree MSc, PhD, MD

Experience

Essential criteria

Full GMC registration at time of appointment Evidence that present achievement and performance is commensurate with totality of training. Evidence of experience in a range of acute medical specialties, with experience of managing patients on unselected medical take, during core training or equivalent.

Desirable criteria

Experience at CMT1/2-level of managing patients with haematological disease and managing haematological emergencies by the time of commencement of ST3 training.

Knowledge

Essential criteria

Demonstrates awareness of the basics of managing patients with haematological disease Competence at core-completion level in the management of medical emergencies, in-patients and outpatients

Desirable criteria

Evidence of skills in the management of acute medical emergencies (e.g. ALERT, IMPACT certification) Evidence of skills in the management of patients not requiring hospital admission Evidence of ST3-level procedural skills relevant to medical patients (clinical independence in central venous cannulation, chest drain insertion, joint aspiration, DC cardioversion, abdominal paracentesis.)
