Job summary

An exciting opportunity has arisen for an enthusiastic, ambitious, and innovative individual to join the Central Cluster Support team, aligned with the Conwy West cluster area.

Primary and Community Care is an exciting place to work right now. The Accelerated Cluster Development (ACD) programme presents opportunities to change the footprint of our system, to bring services closer to the patient, in a joined-up approach.

Main duties of the job

An integral part of the Central (Conwy & Denbighshire) Integrated Health Community (IHC), your work will ensure primary care and wider community services are developed and deployed effectively, serving the local population in a whole-system approach, so people receive the right care, at the right time, from the right service.

You will work alongside cluster and professional collaborative leads, and IHC members, to develop the roles and functions of clusters and professional collaboratives.

A great facilitator and communicator, you will be responsible for providing support to clusters and other networks, ensuring they can respond to population needs through effective and robust planning and service delivery, whilst leading development and innovation at pace.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
