Due to internal promotion..... The Pendle Medical Partnership is looking for a motivated, friendly, committed and enthusiastic Care Navigator with great customer service and administrative skills,to join our busy, but friendly Practice Team.

The position is part-time, based on 20 hours per week, working across our Earby & Colne sites.

We envisage the following working pattern:

Monday 1-6pm COLNE

Tuesday 8-1pm COLNE

Wednesday 1-6pm EARBY

Friday 1-6pm EARBY

Some Saturdays mornings throughout the year are required.

Experience of working in a GP Practice, healthcare setting or customer services is preferred but full training and support will be given to the successful candidate.

Main duties of the job

The successful candidate will support the practice team by signposting patients to appropriate healthcare professionals or services (care navigating). You will also undertake reception duties. This is a varied role that can be challenging and rewarding, but different every day.

You will be required to undertake a variety of tasks that support our patients and clinicians who deliver the best care to our patients. The role includes dealing with patient enquiries, both in person and on the telephone, navigating patients to the most appropriate care provider, handling computerised patient information as well as providing general assistance to the wider practice team. We work from two sites - The Surgery at Earby, and Floor 2 within Colne Health Centre. As one of the largest practices in East Lancashire, we can offer an extensive range of services, enabling our patients to be cared for in the community, often avoiding the need for long waits for hospital services.

With 10 GPs, 7 experienced practice nurses, 2 Physician's Associates, 1 Clinical Practitioner, our own pharmacist, 2 Nurse Associates, GP Assistant, Health Care Assistants and Care Navigators with excellent knowledge of services in this area, we believe that we are well equipped to support you with your health and care needs.

Job description

Job responsibilities

The Primary focus of the role is to assist and direct patients to the most appropriate service or health care professional (care navigating) in a courteous, efficient and effective way. Process appointment requests by telephone and in person using the EMIS Web clinical system. Work in accordance with the Practice protocols, policies and procedures. Deal with admin queries via the e-Consult system. Deal with general enquiries and make follow-up appointments. Handling and processing prescriptions. Deal with requests for home visits. Process e-mails received into the Practice. Register new patients on to the clinical system. Process patients change of address on the clinical system. Respond to tasks and queries from the practice team. Input patient data into the clinical system. Scanning of patient information and documentation. Advise patients of relevant charges for private services and accept and process electronic payments. Deal with conflict and work with patients to ensure their needs are met in line with practice protocol. Ensure that the reception and waiting area is kept clean and tidy. Provide additional cover for annual leave and sickness absence. Any other tasks allocated by Managers or clinical staff. Open or lock up the building when required. Work occasional Saturday mornings. Experience

Essential

Experience of working in a busy office environment. Experience of dealing with confidential information. Experience of working in a GP Practice, healthcare setting or a customer service setting that involved direct patient/customer contact.

Desirable

Knowledge of the EMIS system. Experience of dealing with vulnerable people. Experience of working in a team setting.
