Do you wish to nurse where patients and families come first? Palliative care is a dynamic and evolving speciality. Working within a Hospice setting, the post is ideally suited to Registered Nurses who will bring clinical experience and knowledge gained within a general or acute medical ward or an oncology setting. It is desirable that the post holder will be both experienced and competent in venepuncture and cannulation, IV Drug Administration and Care of Venous Devices.

Job Purpose: The post holder is responsible, as part of the multi-disciplinary team, for the assessment of care needs, the planning and development of nursing programmes of Specialist Palliative Care/Hospital Based Complex Clinical Care and/or the implementation and evaluation of these programmes. 

The post holder is expected to carry out all relevant forms of care without direct supervision and may be required to demonstrate procedures to supervise qualified and/or unqualified staff including basic and/or post basic learners. The post holder may be required to take charge of the ward/department as a necessary part of role development.

Duties and Responsibilities

It is the responsibility of the Staff Nurse / Senior Staff Nurse to:

Ensure the implementation of the Hospice Core Values and Mission in practice. Demonstrate awareness of the importance of etiquette and good conduct within the Hospice environment. Ensure an effective delivery of nursing care is provided to patients and that the highest standard of care is maintained. Provide, through Hospice policies, a safe environment for the treatment of patients and protection of staff. Participate in education and training programmes as required. Participate in rotation and redeployment of staff as required. Implement all policies, procedures and practices as introduced at Induction, Mandatory Training and throughout mentorship experience. Undertake all duties in accordance with legislation, Hospice philosophy and recognised professional standards and guidelines.

Principal Functions

Nursing Care

Accountable for their own practice and development. Carries out allocated duties efficiently and effectively and, where appropriate, delegates and ensures that other members of the nursing team provide nursing care to agreed standards. Responsible for the safety of patients and ensuring that safe working practices are maintained within the Unit. Ensures that accurate records of care are maintained according to NMC standards, Hospice Policy and Protocol and in line with legal requirements. Implements the highest standards of infection prevention and control, following guidance as per the National Infection Prevention and Control Manual, the Hospice Infection Control Manual including Specific Infections, the Ward Manager and Senior Nurse Management Team to monitor safe usage of PPE and safe working practices.

Management

Exercises positive leadership and achieves a high standard of care delivery based on sound organisational objectives and effective working relationships with all grades of staff. Ensures all care and support are underpinned by the Health and Safety at Work Act (1974) including continuous risk assessment. Ensures all care and support provided to patients and families is reflective of good governance. Participates in clinical audit as per the Hospice Quality Improvement Programme. Ensures all clinical practice is of the highest standard following clinical policies, procedures and guidelines at all times, whilst ensuring nursing staff are aware of and adhere to Hospice policies for example, correct moving and handling techniques, infection prevention and control and the safe use of equipment. Participates in the moving and handling of patients in accordance with current guidelines, competence assessment and good practice. Ensures all staff are conversant with the fire regulations of the Hospice and within each area including the site of appliances and exits. Maintains an effective communication system to ensure the dissemination of information affecting all aspects of the delivery of patient care. Respects the confidentiality of all aspects of patient care, sensitive information and matters of Hospice business. Works with members of the multi-disciplinary team to meet the needs of patients and their families and also maintains effective contact with patients, relatives and visitors. Works closely with other Hospice Teams to ensure the delivery of safe and effective care Hospice Maintenance Team, Garden Room Team, Housekeeping Team, Hostess Team, Laundry Team and Administration Team. Politely welcomes patients and is courteous when approaching visitors accessing the Hospice. Coordinates the admission, transfer and discharge of patients reaching out to external teams where appropriate. Takes charge of the ward in the absence of Senior Staff to maintain the smooth running and efficiency of the ward. Cooperates with Senior Staff to ensure that all efforts are made to safeguard the welfare and safety of patients, relatives, and staff. Carries out all duties with no/minimal supervision in a safe self-directed manner. Ensures allocated tasks are carried out and that agreed standards within the Unit are maintained. Counsels junior staff where appropriate, under the direction of a senior member of staff. Participates in the assessment of junior staff as per the Hospice Assessment/Appraisal Programme Identifies potential problems emerging within the Unit and promptly reports to a senior member of staff. - Urban Practice
