Specialist Pharmacist Progression Post

Permanent- per week

We are looking for an enthusiastic, highly motivated, Registered Pharmacist with demonstrable post qualification experience, who wants to make a real difference to patient care, to join our successful team and contribute towards developing our services.

You will work with the Pharmacy Team in providing and developing an efficient and comprehensive pharmaceutical service for patients. To support advanced pharmacists in the implementation of medicines management plans. To be professionally accountable for any pharmacy function for which they assume responsibility.

The initial banding of the postholder will depend on past experience and/or completion of a conversion competency document.

Main duties of the job

Band 6 Pharmacists participate in our training rotation providing a wide variety of experience including surgical, acute admissions and emergency medicine, orthopaedics, paediatrics. Whatever your ambitions, our personalised clinical training programme will develop your medicine optimisation skills.

Band 7 Pharmacists have sufficient experience not to require direct professional support and will participate in a range of routine professional and clinical services provided by the department, but will also be allocated to a specific service(s) as part of their duties for example Aseptics, Anticoagulation Clinics, Intermediate Care Services, Medicines Infomation, Palliative Care, Clinical Trials, iQemo, Trainee Pharmacist tutor and Immunoglobulins.

The pharmacist may accompany a Consultant on his / her regular ward rounds; attend Pharmacist-led clinics. The Pharmacist will be required to take an active part in new service development. The Pharmacist will spend part of their time allocated to one or more specific and specialist areas of pharmaceutical activity supporting the work of the Advanced Clinical Pharmacist(s) who have day-to-day responsibility for these activities

Participation in the extended hours, weekend, Bank Holiday and on-call service will be required

Job responsibilities

SECTION 1- MAIN RESPONSIBILITIES :

Clinical Services

Participates in ward and clinical pharmacy services; contributes to the development of the pharmacy care record; and provides advice to ensure safe and cost effective prescribing and drug expenditure control.

Compiles a drug history where possible from patients admitted to hospital to ensure continuity of their medication and records relevant information in order to facilitate the discharge process. This involves communication with the patient, their GP and hospital medical staff and reference to the patients notes in order to obtain accurate details of medication, allergies and other relevant clinical information. This will involve checking of the patients own medication and assessing its suitability for use where appropriate

Produces , reviews and updates pharmaceutical care plans depending on clinical response , results of investigations and adverse effects

Monitors in-patient and discharge prescriptions for clinical accuracy and cost effective

Assesses the patients response to therapy, interprets blood results, and depending on their clinical parameters either adjusts drug doses in accordance with local procedures or advises the clinicians appropriately.

Advises on therapeutic drug monitoring as appropriate to ensure patients receive optimum and effective treatment.

Monitors and reports adverse drug reactions that patients experience as a result of their medication in line with national guidance.

Participates on consultant ward rounds where applicable.

Documents clinical interventions and activities in line with local policy.

Participates in specialist inpatient and outpatient clinical pharmacy services in accordance with local provision and works with healthcare staff to ensure the efficient operation of these services.

Uses inter-personal skills to counsel patients to ensure they gain maximum benefit from their medicines, and educating them regarding the use of equipment for the administration of medicines.

Band 7- Clinically responsible and accountable for providing a rapid discharge service to facilitate the early discharge of patients from the clinical area Participates in the teaching of specific patient groups about their medicines. Band 7- To participate, as required , as a specialist pharmacist in the pharmacist-led anticoagulant This will include reviewing patients by appointment in the clinic and undertaking Patient interview. Patient education. Consideration of disease state , co-existing illness Interpretation of laboratory blood results. Prescribing of anticoagulant dosage Administration of anticoagulation reversal when clinically appropriate. Co-ordination of emergency care. Liaison with other health professionals and carers

Medicines Optimisation

Promotes adherence to the Trust Medicine Formulary, treatment guidelines and protocols.

Assists in drug utilisation and expenditure reviews and clinical audit activities.

Initiates investigations into problems arising from the use of pharmaceutical products and assists in the development of new methods of service delivery.

Liaises with medical and nursing staff to ensure medication is prescribed and administered correctly in accordance with trust policy , to maximise patient safety

Dispensary

Participates in dispensing services including clinical and final accuracy checking of prescriptions / orders for inpatients, outpatients, patients being discharged and mental health patients on leave in accordance with local standards, policies and safe systems of work and to be professionally / legally responsible for this work.

Supervises work carried out by other pharmaceutical staff as required ensuring compliance with agreed legal and professional standards.

Counsels outpatients to ensure that they gain maximum benefit from their medicines.

Assists medical staff in the undertaking of approved clinical trials by arranging the necessary supplies and aspects of trial protocol affecting pharmacy services in accordance with ICH / GCP guidelines & European Directive.

Ward Distribution Service

Liaises with other pharmacy staff providing a supply service to wards in order to maintain safe and economic stock levels. Ensures appropriate storage of pharmaceutical products on wards and departments and that these products are suitable and fit for use.

Undertakes or takes responsibility for ensuring that the Controlled Drug stocks on their designated wards are checked by Pharmacy staff on a scheduled basis

Provides safe and secure transport of medicines to and from the wards as necessary.

Medicines Information

Provides information and advice on all aspects of pharmaceutical products as requested by medical & nursing staff and other health professionals including other Trust professional staff, GPs, community pharmacists, and residential care and nursing home staff.

Provides information and advice to patients on all aspects of their medicines

Undertakes critical appraisal and evaluation of literature relating to the use of pharmaceutical products.

Band 7- Aseptic Dispensing Service

Clinically checks and validates prescriptions received from wards for aseptic products for individual patient including drug / dose / appropriateness for CIVAS items; protocol / dosing / dose intervals for chemotherapy; advice on appropriate TPN therapy and subsequent variations.

Ensures the safe operation of the aseptic facilities and correct staffing appropriate to workload during the period of supervision of the unit.

Is responsible for handling queries on preparation details / stabilities / calculations etc and forwarding to the appropriate persons as required.

Is responsible for undertaking the technical check and final release for individually dispensed aseptic products prepared under Section 10 exemption of the Medicines Act, including adult and paediatric intravenous additives, total parenteral nutrition and cytotoxic chemotherapy in accordance with the Guide to Good Manufacturing Practice, local procedures and policies, and safe systems of work.

Liaises with Doctors, Nurses and other Health Care Professionals regarding the supply of Aseptic Products.

Education & Training

Participates in the clinical training, mentoring and supervision for Trainee pharmacists, less experienced pharmacists, pharmacy technicians and pharmacy undergraduates.

Provide individual assessments and testimonials for Trainee pharmacists as evidence of their training

Participate, as necessary, in the training of medical , nursing and other hospital staff in medicines management, either within or external to the Trust

Undertake and supervise practice research and clinical audit.

Undertakes a course of study leading to a diploma / MSc in pharmacy practice where appropriate.

Keeps abreast of current pharmaceutical practice and knowledge, attending meetings, courses or conferences as necessary, for continuing professional development in accordance with national guidelines from the Royal Pharmaceutical Society.

General Duties

Maintains a high standard of work and promotes the professional image of Pharmacy within and outside the department.

Ensures that safe working systems are in operation in accordance with Trust and departmental procedures.

Ensures that all legal requirements relating to the prescribing, dispensing and storage of pharmaceutical products in the Trust are met and are in accordance with criteria set out under COSSH regulations.

To participate on a rota basis in Saturday, Sunday, Bank Holiday and late night duties as is considered appropriate by the Chief Pharmacist or other senior manager and in line with current local policy.

Participates in the provision of on-call duties in accordance with local procedures. This involves the provision of advice and information relating to the treatment of patients and the supply of urgently required pharmaceutical products outside of normal working hours. This may require arranging supply of pharmaceutical products from other hospitals or wholesalers. Takes appropriate action following receipt of Drug Alerts outside of normal working hours in accordance with local procedure.

Holds a set of keys for the pharmacy department with associated responsibility for the security of drugs and equipment.
