Job summary

To work as part of theIntegrated Care Team providing direct care to children and young people whohave palliative care needs, in the hospice, home or hospital as directed by thechild's care needs and service delivery. The post holder will be required to work a variety of shifts as thehospice provides care services 24 hours a day, 365 days a year

CORERESPONSIBILITIES

Clinical

1.To provide high quality nursing care for children andyoung people who have palliative care needs.

2.To promote a person-centred care approach within themulti-disciplinary team.

3.To contribute to the care plans and assessmentsrequired to ensure the child/young person receives safe and effective care fromhospice services.

4.To liaise with the lead for mental capacity assessmentsand ensure that these are competed as required.

5.To ensure care is planned and delivered in accordancewith the Care Quality Commission Inspectorate standards of quality and safety.

6.To be fully conversant with and contribute to allclinical guidelines, policies and procedures of Haven House.

7.To communicate complex and sensitive informationeffectively to service users, care staff and other members of the team.

8.To further develop clinical skills and knowledge andmaintain own competency for NMC portfolio.

9.To work with the Nurse Practice Educator in deliveringtraining and further support junior colleagues with responsibility towardshealth care support workers.

10. Toensure that safeguarding is embedded in practice and concerns are escalated atthe earliest opportunity.

11. Tocontribute to effective medicines management in accordance with Haven Housepolicies.

12. Todevelop and maintain clinical skills and knowledge necessary to provideholistic evidence-based nursing care.

13. Toact as an advocate for the child and family.

14. Toensure that the child/young persons nutritional needs are met whilst at HavenHouse.

15. Tohave a knowledge of the commonly used drugs in paediatric palliative care andto undertake the relevant drugadministration competencies.

16. Tohave a knowledge of safeguarding policies and procedures.

17. Touse Haven Houses Electronic Patient Record System to ensure data is entered inan accurate and timely fashion, in accordance with policy and procedure.

18. Todemonstrate a professional approach to work and act in accordance the NMC Code.

Work collaboratively with otherteam members to: -

1. To constructively identifyissues with care strategy, policies and procedure in the interests of theorganisation and users of the service.

2.To contribute in the evaluation of services as requiredensuring that children, young people and family's views are represented.

3.To be aware of allocated resources and ensure effectivetime management in own role.

Clinical Governance

1.To participate in audit, service user feedbackcollection and research as required.

2.To report clinical and non-clinical incidents inaccordance with policy.

Educational

1.To identify own learning needs and ensure professionaldevelopment is maintained and contributes to the appraisal system.

2.To attend all mandatory training as required by theservice.

3.To mentor pre-registration nursing students, providingfeedback to students assessor.

4.To contribute to overall staff and service developmentby updating the evidence base for the team re clinical practice.

5.Education and training of parents/carers to meet thehealth care needs of their child.

6.Education and training of junior staff and students tomeet the health care needs of service users.

7.To comply with Haven Houses arrangements forcontinuing professional development.

Professional

1. To adhere to Haven House policies and NMCCode of standards of conduct, performance and ethics for nurses and midwives.

2. To keep clear and accurate records.

3. To maintain confidentiality at alltimes in conjunction with Data Protection and Caldicott guidance.

Other duties

1. To promote and live the HavenHouse values of Compassionate, Professional, Respectful and Safe.

2.Towork with colleagues across the hospice as part of the Haven House One Teamapproach.

3.Toconduct yourself in line with the Haven House cultures and behaviours at alltimes, acting as an ambassador for the hospice.

5.Tooffer flexible support for the work of the organisation, with a flexibleapproach to new duties and responsibilities as needed to support our vision,mission and values.

RN Child / Adult / LD

ENB 998/Teaching/Mentoring qualification Palliative Care /Symptom Management in Life Limiting Illness or similar module

Experience

Essential

Experience of working independently as well as part of a team. Ability to teach clinical skills to parents/care staff. Ability to work alone in a variety of settings. Self-motivated. Literate and Numerate in English. IT/Computer literate: able to word process and use basic spreadsheets. Flexible and adaptable at work in order to meet competing priorities. Understanding of a range of paediatric conditions including life. limiting, life threatening, and end of life care needs. Understanding of infection control procedures. Understanding of personal health and safety responsibilities. Good understanding of the meaning of clinical supervision/mentoring. An understanding of equality within the workforce and how it can impact on this post. Able to work unsocial hours as required.

Desirable

Experience of a broad range of paediatric nursing needs and complex needs care, technology dependent child, childhood cancer. Experience of caring for children/young people who require end of life support. Technical paediatric nursing skills such as care of enteral feeding tubes, tracheostomies, emergency medication for seizures etc. IV competent. Ability to teach clinical skills to parents/care staff. Understanding of a range of paediatric conditions including life limiting, life threatening, and end of life care needs. Access to vehicle and clean driving licence.
