Job summary

Salus is a GP Federation which was formed bythe local GP practices within North East Hampshire and Farnham. This Includes practices located in Aldershot,Farnborough, Fleet, Farnham & Yateley.We hold several contracts supporting primary care to deliver services onbehalf of those practices, helping them work better together and with otherhealthcare providers leading to improved patient services and developing newones due to constant changes in the Primary Care environment.

We also assist practices with admin,operational and HR support and facilitate shared learning acrosslocalities. By engaging with the public,we also help ensure that services are designed and developed with patient andpublic involvement.

Main duties of the job

Clinical& psycho-social triage, supported by robust protocol and clinical templates.This will include a high level of rapid, fast and routine pace communication,over the telephone, virtually via Microsoft Teams and by email.

Supportand input to care planning and collaborative decision making alongsidecommunity service colleagues including Urgent Community Response Teams,Intermediate Care Teams, Community Nurses, Mental Health Practitioners,Specialist Nurses, Community-Based Therapy Teams, Social Prescribers, AdultSocial Care, Hospice Services and Home Visiting Paramedic Practitioners

Thisrole is for 16 hours per week, this will be on set days as follows: Monday 12pm 8pm; Tuesday 8am 12pm & one other morning (8am 12pm) of your choice.

Job description

Job responsibilities

This role willsolely involve the triaging of referrals into the community system and IntegratedCare Team via the Community Single Point of Access.

This will include:

Clinical andpsychosocial triage of incoming referrals and points of contact from a broadrange of practitioners; and patients Collaborativejoint triage, where appropriate, with colleagues across health and social care Telephonepsychosocial triage of patients over the telephone, where appropriate, toestablish baseline needs and urgency. Chairing &coordinating the daily multidisciplinary virtual huddle. This short meetingconsists of duty practitioners across core stakeholder partner organisations,where effective communication and decision making is platformed to supportthose patients at highest risk of possible admission Liaise with relatives, carers, and health careprofessionals to prevent and minimise the risk ofacute hospital admission where appropriate, working in partnership withsecondary care discharge co-ordinating services. The post holderwill: Be autonomous inpractice Ensure resourcesare utilised in a cost-effective manner Ensure allprofessional/clinical practices are in line with Salus policies Ensureappropriate and accurate records are maintained Be accountablefor own professional actions according to NMC Code of Professional Code Maintain your ContinuousProfessional Development according to NMC Code of Professional Code andRevalidation requirements Person Specification

Qualifications

Essential

A) Qualified Nurse with wider experience to include a Community or Secondary Care role, or experience of community care coordination within a Primary Care Setting, is preferable, but not essential. B) Excellent communication skills and the ability to work as part of a team. C) Previous experience of EMIS is desirable, but not essential.
