As a representative of the service, the post holder will be a source of expertise and advocacy to enable other services to better understand the needs of people with long term health conditions or atypical healthcare utilisers. This will include linking with commissioners and trust leads to report on the benefits to patient care, recognising where partnerships may help to improve the lives of patients and developing pathways to improve the patient journey.
