This Advanced Nurse Practitioner role will involveworking alongside our multi-disciplinary team which consists of a GP Partner,Advanced Nurse Practitioner Partner and Pharmacist Partner. They are allsupported by Advanced Nurse Practitioners who lead on acute same day care,alongside the Home Visiting Team and First Contact Physio, Pharmacists,Practice Nurses and HCAs. We also have Social Prescribers who work as PCNstaff dealing with the Mental Health of our patients.
The successful Advanced Nurse Practitioner willwork as an autonomous practitioner, diagnosing, managing, prescribing anddischarging patients with illness or injury across all age groups includingyoung children. This may be by face-to-face consultation, telephone or videoconsultation.

Main duties of the job
The AdvancedNurse Practitioner (ANP) has been assessed as competent in practice using theirexpert clinical knowledge and skills. They have the freedom and authority toact and make autonomous decisions in the assessment, diagnosis and treatment ofpatients.
The ANPprovides an opportunity for patients to avoid the need to be referred to a GP,thereby enabling them to receive timely care and negate unnecessary delay inreceiving treatment.
ANPs in primarycare can develop close, long-term relationships with their patients and work inpartnership with them to achieve optimum health. They are autonomous in makingdecisions based on assessment, diagnosis and interpretation of test results. Theycan independently prescribe appropriate medication, evaluate or refer to otherspecialists if necessary.

PrimaryResponsibilities:
The followingare the core responsibilities of the Advanced Nurse Practitioner. There may be,on occasion, a requirement to carry out other tasks; this will be dependentupon factors such as workload and staffing levels:
Develop,implement and embed health promotion and wellbeing programmes
Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
Implementand evaluate individual specialised treatment plans for chronic diseasepatients
Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
Requestpathology services as necessary
Processand interpret pathology and other test results as required
Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
