Job summary

TheLakes Medical Centre is looking for an experienced Pharmacist to join ourfriendly and successful practice team. Weare a forward-thinking training practice, supportive of continued education,personal development and focussed on providing high quality patient care to10,800+ patients.

Main duties of the job

Reduceinappropriate poly-pharmacy and wasteful prescribing through clinicalmedication review. Reconcile medicines following hospital discharge and workwith patients and community pharmacists to ensure patients receive themedicines they need post discharge.

Contribute toreductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines related issues.

Managepatients and health care professionals medicine queries.

Managepractice formularies to improve the quality, safety and cost effectiveness ofprescribing.

Implementdrug withdrawals and alerts MHRA aimed at improving medicines safety.

Work withprimary care professionals and patients to implement NICE and other evidencebased guidelines.

Run clinicswhere medicines are the main interventions.

Contribute tomulti-morbidity reviews and multidisciplinary reviews.

Providemedicines information and training to Practice healthcare professionals andadmin staff.

Act as asource of medicines information for all of the practice team and patients (around doses, side effects, adverse events, possible alternatives aroundout of stocks)

Working within the practice based team to undertakemedication reviews particularly in high risk groups.

Improve prescribing practice through educationalsupport for all prescribers within the practice.

About us

Our premises also house Community mental health and Social prescribing clinics. We practise from purpose-built premises with easy access to Manchester city centre and the North West motorway network. As an established training practice, we are proud to have trained GP trainees, Advanced Nurse Practitioners and Foundation year doctors.

We are actively involved in our local PCN and have additional staff including PCN phamacists and pharmacy technicians that make up our medicine management team.

Job responsibilities

Lead on where changes in evidence require changesin prescribing across patient population where a drug is withdrawn orindications change.

Liaise with colleagues in community pharmacy toalign support for medicines adherence.

Increasequality and safety and support improvements in clinical care through practicebased audit and implementing change.

Prescribing advice to prescribers in practice temporary non availability of drugs.

Ensure patient safety when they are transferredbetween care providers through reconciliation of prescribed medicines.

Demonstrate general pharmaceutical skills andknowledge in core areas.

Demonstrate accountability for deliveringprofessional expertise and direct service provision as an individual.

Recognise priorities when problem-solving andidentify deviations from the normal pattern and refer to GPs when appropriate.

Follow legal, ethical, professional andorganisational policies/procedures and codes of conduct .

Involve patients in decisions about prescribedmedicines and support adherence as per NICE guidance.

Recognise the roles of other colleagues within theorganisation and their involvement in patient care.

Demonstrate use of appropriate communication togain the co-operation of relevant stakeholders (including patients, senior andpeer colleagues, and other professionals, other NHS/private organisations, ICB).

Demonstrate ability to work as a member of the team

Recognise personal limitations and refer to moreappropriate colleague(s) when necessary.

Liaise with ICB colleagues including ICB medicine management team on prescribing related matters to ensure consistency of patientcare and benefit.

Demonstrate understanding of the pharmacy role ingovernance and implement this appropriately within the practice.

Demonstrate understanding of, and contribute to theworkplace vision.

Demonstrate ability to improve quality withinlimitations of service.

Demonstrate ability to motivate self to achievegoals.

Oversee the MHRA alerts/CAS alerts to make surethese are audited, actioned and logged on a spreadsheet

Conduct regular medication audits under theguidance of the Prescribing Lead GP.

Assist in training of Pharmacy technicians andtrainee doctors.

Basis: Part Time 25 hours (negotiable)

Qualifications

Essential

Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council. Minimum of 2 years post graduate experience in pharmacy. May hold or be working towards a postgraduate pharmacy qualification Demonstrate self-development through continuous professional development activity.
