Job summary

The Hollies Medical Practice has an exciting opportunity available for an Advanced Practitioner (we would consider applicants from advanced nurse practitioners or other experienced clinical practitioners, such as paramedic practitioners) to join our team.

The Holliesis a friendly, high achieving practice based on the edge of Tamworth towncentre and provides care to 15,500 patients.

If you join us at the practice, you will:

Be part of this supportive team, which also includes 8 GP partners, 2 salaried GPs, 3 clinical pharmacists, a pharmacytechnician, 1 senior practice nurse, 4practice nurses, 4 healthcare assistants, 1 nurse associate and 2 care coordinators. Wealso have access to social prescribers via thenetwork. In addition to the clinicalstaff, we have a strong management and administrative team.

The Hollies was ratedgood at the last CQC inspection and in the national GP patient survey for 2020,90% of the patients surveyed described their overall experience of the practiceas good (compared to CCG average of 80% and national average of 82%).

Main duties of the job

The successful applicant will be an experienced advanced clinical practitioner, who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will have experience of minor conditions and managing patients with long term conditions. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

An interest in caring for palliative patients and residents of care homes is desirable.

About us

This is a full time post ( hours Monday to Friday between the hours of 7am - a rota basis, occasional Saturday mornings are treated as overtime).Flexibility is required.
