Job summary

We have an exciting opportunity for a Nursing Associate to join our Primary Care Network team. The Nursing Associate will work under the supervision of the Practice Nursing Team within the allocated PCN.

Leadership and support will be provided by the PCN Manager, Clinical Lead, and member practices. The Nursing Associate will work within the community to support the everyday running of the PCN practices.

Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience.

The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.

Main duties of the job

Work within the PCN to provide and monitor care, alongside the wider PCN team, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision

Improve safety and quality of care at every opportunity

Undertake additional clinical training as required by the PCN to support the Network Contract/DES Services Specification

Support registered nurses and the wider team to enable them to focus on the more complex clinical care

To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience

Provide support to the PCN with regards to their Network Contract DES Services Specification including CVD prevention and tackling health inequalities

Develop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health services.

Job responsibilities

Accurately perform and record clinicalobservations such as blood pressure, temperature, respiration, and pulse rates.

Engage positively with patients/carers/familiesand staff members promoting mutually respectful and appropriate relationships.

Communicate effectively with patients regardingproviding and planning their care and carrying out risk assessment needs asrequired.

Communicate complex and sensitive informationverbally and in writing. Adapt and tailor communication skills to the need ofthe listener.

To act as patient advocate during practice orPCN meetings, sharing any relevant information confidently and accurately,maintaining patient confidentiality appropriately.

Develop an understanding of MDT structures andmembers roles.

Support patients to improve and maintain theirphysical, mental, and behavioural health and well-being.

Provide advice to patients including promotingself -management where appropriate and signposting patients to other PCNcolleagues, local community, and voluntary sector services

Maintain accurate and contemporaneous patienthealth records.

Work in adherence to NMC Code for NursingAssociates, within own level of competence and the protocols and policies ofthe practices/PCN.

Work flexibly to meet the needs of the serviceand patients. Adapt to what may be potentially challenging and changingsituations at times.

Willing to undertake other duties deemedappropriate to a Nursing Associate following correct training/guidance andagreed by the post holder and their line manager.

The post-holderwill participate in training programmes implemented by the practice as part ofthis employment. This will include:

Participation in an annual individualperformance review, including taking responsibility for maintaining a record ofown personal and/or professional development.

Taking responsibility for their own development,learning and performance. They may be involved in demonstrating skills andactivities to others who are undertaking similar work.

The Nursing Associate must continue withrevalidation in accordance with NMC guidelines

Complying with all PCN protocols in relationto GP practice administration and clinical practice.

Adhere to the relevant NHS Codes of Practice andown professional Code of Conduct.

It is expected that the post holder willundertake any other duties which are deemed appropriate to the band/role whenrequested by GPs/other clinical team members.

Qualifications

Essential

Specific qualification and training requirements as specified in the Nursing and Midwifery Council Standards of Proficiency by having undertaken and completed the two-year foundation degree, recognised by the Nursing and Midwifery Council Registered with the NMC and revalidation is undertaken in line with NMC requirements.
