Main duties of the job
The Lead Nurse will provide clinical and professional leadership to the nursing and healthcare assistant team. The post holder will ensure nursing services are delivered effectively to the patient population.
Our ideal candidate will have drive, dedication, enthusiasm, and a positive attitude. They will provide effective and high standard care to patients including, all chronic disease management, wound care, cervical cytology, ear care and other duties.
The post holder will line manage the nursing and healthcare assistant team, ensuring high quality patient centred care is delivered and facilitate their professional development.
The Lead Nurse is responsible for leading and directing the development of new services and patient care pathways, ensuring they match the needs of patients.

It is an expectation for this post holderto assess their own learning needs and undertake learning as appropriate.

Job description
The followingare the core responsibilities of the lead practice nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels: As the Clinical lead, you willhave overall line management responsibility for the Practice Nursing TeamProvide clinical leadership to the nursing team Mentor new nursing staffSupport the development of others to maximise staff potential in the team.Identify the potential for service developments, risk and deficits and informpractice manager making recommendations based on specialist knowledge Undertakeone-to-ones, joint appraisals, regular Practice Nurse and HCA meetings,clinical education, and other practice-wide learning events and meetings.
To provide routine nursing care to patients as required in accordance withclinical based evidence, local area prescribing guidelines NICE. Play a keyrole in our clinical governance and overall practice management team. Act as aclinical leader in the delivery of practice nursing services to patients,ensuring that the needs of the patient are the priority. Attend regular nursingmeetings. Lead the nursing team in the planning and implementation of localguidelines, protocols, and standards, as well as local projects or initiatives. Ensure health and safety and infection control are high priorities, supportedby other members of the team. Work with our Practice Management team to ensurewe monitor practice performance of QOF and enhanced services regularly toachieve high standards of patient care. Take responsibility for owndevelopment, learning, and performance, including organising clinicalsupervision where appropriate and acting as a positive role model. Work tosupport staff wellbeing and development. Quality improvement Meet CQC standards Work with colleagues and management teamto ensure that we focus our clinical time to maximum benefit of patients andthat our systems and processes support staff to focus on what matters most. Tooversee the provision of wound care services to patients. To co-ordinate theprovision of travel medicine services. To maintain accurate clinical records inconjunction with existing legislation. To ensure read codes are usedeffectively by all members of the nursing team. To prioritise health issues andintervene appropriately. To support the team in dealing with clinicalemergencies. To implement vaccination programmes for adults and children and tolead on vaccine updates / ensure current national vaccine guidelines are beingfollowed and develop and maintain vaccine protocols. To order imms and vaccs,to liaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.) To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual) To support the clinical team with all safeguardingmatters in accordance with local and national policies. To deliveropportunistic health promotion where appropriate. To check/ update PGD anupdate protocols (with support from GPs and Practice Management) To be responsible for ensuring maximum optimisation of use of appointmentsacross the nursing coordinate and organise HCA clinics, training andpersonal development.
