Job summary

You will be responsible for leading the nursing team on a day to day basis; coordinating the delivery of holistic patient care within the clinical environment, whilst supporting flow in terms of admissions, transfers and discharges to multiple points of contact.

Through clinical leadership, adherence of the Trust Right Care values and utilisation of effective management skills, Senior Sister/Charge Nurse will ensure the department team is informed, trained and developed to enable the highest standards of evidence based and compassionate care to be delivered to our patients, their carers and families. To support a collaborative, open and trusting communication style to all groups that the Unit communicates or interacts with from staff, to visiting teams and the individuals we care for.

To actively participate in the development of the ward using innovation, challenge and evidence based practice/strategies to support forward growth, progression and value based care.

You must understand the core concepts of safe and effective patient flow through the Department and beyond the acute hospital, using their skills to promote and support the multidisciplinary team.

Main duties of the job

Provision of care

The post holder will coordinate the department and take responsibility and accountability for the delivery of care to patients who present to the department. To have the clinical skill to support or identify and evaluate factors which contribute to the complex nature of patient presentations, determining the nature and urgency of the patient's clinical and personal needs. Determines and plans the range and sequence of assessments that are appropriate to the presenting patient group, using where appropriate Trust standard operating procedures, guidelines, policies and protocols and any risks that need to be managed. Carry out assessments in line with evidence based practice, legislation, policies and procedures, established theories or models. Monitor individuals and adjust your approach and management in accordance. Considers and interprets all of the information available using systematic processes or reasoning and reach a justifiable conclusion. Support, develop and record treatment plans that are appropriate to the clinical context, consistent with assessment and meets the patient's needs and wishes. Take responsibility for the coordination and monitoring of the efficient delivery of treatment plans. Ensure junior staff are supervised as necessary and supported.
