Job summary

SouthDerbyshire Dales is located on the edge of the beautiful Peak District NationalPark and comprises of 4 GP practices. Each practice has its own PCN ClinicalPharmacist and dedicated Pharmacy Technician support. This means that you canwork closely with an individual practice and build strong relationships withthat practice team.

We are seeking to replace one of our Clinical Pharmacists to support medicines optimisation work at Henmore Health (Clifton Rd Surgery and Brailsford Medical Centre). The successful applicant will work as part of a multi-disciplinary teamwith plenty of support from PCN colleagues and a real chance to shapeprescribing policy within the PCN.

The postholder will be committed to improving health and putting patients first. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital, and practice staff, and to work effectively as part of a team.

The post will be employed by the PCN but based between the two surgery sites. You will be supported clinically by the senior PCN Clinical Pharmacist and prescribing lead GP in the practice. Administrative support will be through the PCN management team with input from the practice.

Closing date Monday 1st July. Part time or job share up to 3 days per week considered

Main duties of the job

The post holder will:

Work as part of a multi-disciplinary team in a patient-facing role based at Clifton Rd Surgery and Brailsford Medical Centre Support delivery of the network Directly Enhanced Service (DES). Impact and Investment Fund (IIF) and PCN pharmacy team workplan Conduct structured medication reviews to proactively manage patients with complex polypharmacy Provide a referral link for Team Up visiting clinicians for onward management of patients Provide clinical medicines expertise to both patients and the practice team Improve prescribing practices across the PCN Improve service and quality through mechanisms such as clinical audits Assist with delivery of enhanced health in care homes and Team Up initiatives across the PCN. Work collaboratively with other pharmacy colleagues across the PCN footprint, including ICB, hospital and community colleagues, to ensure an effective and integrated approach to medicines optimisation for our population.

Primary care experience is essential. We are keen to hear from pharmacists of all backgrounds and level of experience, whether you have already completed or would like to complete the independent prescriber (IP) qualification.Flexibility, enthusiasm and good clinical knowledge are also essential.

There is also the opportunity to participate in extended access work outside of normal working hours if desired.

Job responsibilities

Undertake structured medication reviews with patients with multimorbidity and polypharmacy, including those who are housebound. Involve patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines. Implement own prescribing changes (if an independent prescriber) and order relevant monitoring tests. Care home structured medication reviewsUndertake clinical medication reviews with patients and implement own prescribing changes (if an independent prescriber) and order relevant monitoring tests. Ensure any changes are communicated to care home staff and the community pharmacy. Work with care home staff to improve safety of medicines ordering and administration. Longterm condition reviews in partnership with primary healthcare colleagues, to implement improvements to patients medicines, including deprescribing. Manage own case load, where medicines have a large component ( medicine titration and optimisation for cardiac conditions, difficult to control hypertensives). Review the ongoing need for each medicine, monitoring requirements and support patients with their medicines taking. Provide telephone support for patients with questions, queries and concerns about their medicines to improve patient and carer understanding of, confidence in and compliance with their medicines. Respond to medicine related queries from GPs, other practice staff and members of the wider healthcare team. Provide follow up for patients to monitor the effect of any changes. Implement computer searches to identify cohorts of patients at high risk of harm from medicines. Responsibility for management of risk stratification tools Eclipse Live on behalf of the practice. Work with patients and the primary care team to minimise risks through medicines optimisation. Report medicines related incidents, contribute to investigations and root cause analyses as well as participating in serious incident investigations and multidisciplinary case reviews. Utilise practice searches to identify cohorts of patients most likely to be at risk of an unplanned admission and readmissions from medicines. Work with the multidisciplinary team to manage medicines-related risk for readmission and patient harm. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. To reconcile medicines following discharge from hospitals, intermediate care or into care homes. Identify and rectify unexplained changes, manage these changes, perform a clinical medication review, including dose titrations and any follow up tests. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients ( those with medicine compliance aids or those in care homes).Work in partnership with hospital colleagues ( care of the elderly doctors and clinical pharmacists) to proactively manage patients at high risk of medicine related problems before they are discharged to ensure continuity of care. Support the processing of complex repeat prescribing medication requests and review medicines reaching review dates; make necessary changes, and ensure patients are booked in for necessary monitoring tests where required. Identify and provide leadership on areas of prescribing requiring improvement. Conduct own audits and improvement projects, present results and provide leadership on suggested change. Demonstrate continuous QI activity focused upon prescribing safety and effectiveness. Identify national and local policy and guidance that affects medication, including MHRA alerts, product withdrawals. Implement changes as necessary. Demonstrate understanding of the pharmacy role in governance and implement this appropriately within the workplace. Provide leadership on person-centred medicines optimisation and quality improvement whilst contributing to the QOF and other enhanced services. Provide support to the PCN Medicines team, working collaboratively with other Clinical Pharmacists and Pharmacy Technicians to deliver the annual team workplan. Provide specialist expertise in the use of medicines, to address both the public health and social needs of the patients in the network. Contribute pharmaceutical advice to the development and implementation of treatment pathways that have medicines components better management of long term conditions, reducing antimicrobial resistance, use of dry powder inhalers. Provide leadership to the practice manager and GPs to ensure the practice is compliant with CQC standards where medicines are involved, developing policies and SOPs as needed. Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Provide training to visiting medical, nursing and other healthcare students where appropriate.Implementation of local and national guidelines and formulary recommendations Monitor practice prescribing against the local health economy's "traffic light" list. Liaise directly with hospital colleagues where prescribing needs to be returned to specialists. Audit the practices compliance against local and NICE guidelines. Provide newsletters on important prescribing messages to improve prescribers knowledge and work with the team to develop and implement other techniques known to influence implementation of evidence, such as audit and feedback. Qualifications

Essential

Mandatory registration with General Pharmaceutical Council. Masters degree in pharmacy (MPharm)or equivalent) Independent prescriber or working towards/intent on gaining independent prescribing qualification Minimum of 2 years post-qualification experience In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. Have experience and an awareness of common acute and long-term conditions that are likely to be seen in General Practice. An appreciation of the nature of GPs and general practices An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients) Computer literate with an ability to use GP clinical systems (desirable) and Microsoft Office packages (essential) Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate Able to work under pressure and to meet deadlines Produce timely and informative reports Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers Work effectively independently and as a team member Demonstrates accountability for delivering professional expertise and direct service provision Is able to recognise personal limitations and refer to more appropriate colleagues when necessary.
