Job summary

We have exciting vacancy for a WTE Senior Dietitian specialising in diabetes and cardiac rehabilitation. This combined dietetic role will provide dietetic support for patients with Type 1 and Type 2 Diabetes seen in the Richmond Diabetes Service in addition to supporting patients referred into the Cardiac Rehabilitation Service. The role also includes providing nutrition support to patients on the inpatient unit at Teddington Memorial Hospital.

This is a great opportunity to develop your dietetic skills, knowledge and experience within diabetes, cardiovascular disease and nutrition support. You will need to be a confident, autonomous practitioner working in the outpatient setting however you will have the support of the multidisciplinary teams, and the dietetic department based at Kingston Hospital. This job will require some travel between sites.

Main duties of the job

The Richmond Diabetes service is a multidisciplinary service commissioned to provide a specialist integrated diabetes service to adults with Type 1 and Type 2 diabetes. The role involves outpatient support for patients with diabetes and structured education for Type 2 Diabetes. The candidate should be able to demonstrate advanced knowledge and experience of diabetes clinical management.

The Cardiac rehab role involves working within the multidisciplinary cardiac rehab team supporting this group of patients in specialist clinics and patient education sessions.

M ain Duties:

To work as a member of the Diabetes Team, providing a comprehensive diabetes dietetic service for patients with Type 1 and Type 2 Diabetes.

Provide Dietetic support for patients with cardiac conditions known to the cardiac rehab service

Provide comprehensive dietetic advice to inpatients aiming to improve nutritional status.

Undertake comprehensive specialist assessments, set patient centred goals, create treatment plans, monitor and evaluate a designated caseload of patients, using evidence based clinical pathways.

Formulate treatment plans and goals in partnership with the patient, clearly communicate your clinical reasoning to justify your recommendations utilising the most appropriate treatment/intervention.

To provide specialist clinical advice, support and educate patients and carers in the management of their condition.

JOB PURPOSE:

Provide dietetic support for patients with Type 1 and Type 2 Diabetes seen in the Richmond Community Diabetes Service. To be responsible for the development and provision of a comprehensive and effective, evidence based dietetic service for Cardiac Rehabilitation patients referred across Richmond. Provide nutrition support for patients on the inpatient unit at Teddington Memorial Hospital. To deliver clinically excellent, person-centred care that promotes self-management and well-being. Independently manage a varied caseload, organising effectively and efficiently regarding clinical priorities and use of time. To undertake all clinical duties as an autonomous practitioner, taking responsibility for organising and planning own caseload to meet service and patient priorities. To work as an active member of the multidisciplinary team within the dedicated service areas and actively contribute to the planning and co-ordination of individual patient plans.

COMMUNICATION

Use counselling and motivational interviewing skills to enable, motivate and empower people with health conditions to make sustainable lifestyle changes. To assist patients and their carers to achieve individualised goals to maintain their physical and psychological well-being.

Communicate and support the patient in understanding and managing aspects of their condition, which may at times be distressing, sensitive and unwelcome, understanding that many issues can influence the care and management of diabetes.

Liaise effectively with all members of the multidisciplinary team/departments/services/external agencies to ensure the delivery of an effective and coordinated approach to service delivery.

To plan, deliver and review treatment programmes by providing timely information and advice to GPs, AHPs and Practice Nurses regarding the treatment plan and dietetic recommendations.

To provide expert advice and support to Primary Care, district nurses and other health providers involved in the management of these patients.

To ensure safe practice through development of a caseload management plan taking into account risk management and the documentation and reporting of critical incidents as outlined in trust policy.

To communicate and liaise with all appropriate healthcare professionals in the multidisciplinary teams with regard to their condition and dietary treatment

Gather information from patients, relatives and other services (from a variety of sources including electronic patient records) and maintain clinical documentation in line with the Code of Conduct.

Attend and participate in team meetings.

Advise and educate patients and carers in a variety of ways which may include suggesting techniques and strategies to manage their condition.

To communicate complex or sensitive information, in an empathetic and reassuring manner to patients and carers, including to those whom english is a second language.

Collate feedback and information from patients/carers and their families to inform service evaluation and associated service improvements.

Be able to communicate appropriately over the telephone, email and Teams

Qualifications

Essential

Degree or postgraduate diploma in Dieteticsx Registered with the Health Professions Council

Desirable

Member of the British Dietetic Association DESMOND educator

Experience

Experience of working as a Dietitian within diabetes either in Acute or Community or both
