Main duties of the job

Provide 121 & group health coaching supportfor people with 1 or more long-term conditions,with the aim of: improving peoples levels of activation, empoweringpeople to manage their own health & improve their health outcomes. Itis vital that you have a strong awareness & understanding of when it's appropriate or necessary to refer people back to other healthprofessionals, when the needs are beyond the scope of thehealth & wellbeing coach role when there is a mental health needrequiring the patient be referred to an appropriately qualified practitioner.Support network initiatives to improve staff wellbeing,including the Better Health at Work & develop strong working relationships withhealth, social, & third sector providers in the Durham Dales, includingGP practices & the local Wellbeing for Life closely with professionals from a variety ofhealth specialties.

About us

You will be working as part of the Bishop Auckland Primary Care Network (PCN) team to support the patients of 3 GP practices (Station View Medical Centre, Auckland Medical Group and Bishopgate Medical Practice).The PCN is recruiting a number of new roles to support the wellbeing of patients in this mixed urban & rural area, & you will be part of this proactive & expanding will work flexibly, supporting patients through 121 & group sessions delivered in GP practices, patients homes, & via online video PCN is committed to training & development of all staff & you will be able to develop your career in line with your personal & professional objectives. They will be proactive with a flexible attitude, keen to work as part of a team & committed to developing themselves in this skilled role & will support colleagues to do the flexibly, adapting to the needs of the service & client group.

Job responsibilities

Healthand Wellbeing Coaches support people to take proactive steps to improve the waythey manage their physical and mental health conditions, based on what mattersto them. They support people to develop their knowledge, skills and confidence or to build their patient activation - in managing their health and care,to improve their health outcomes and quality of life. Health coaches do this bycoaching and motivating patients through multiple sessions to identify theirneeds, set goals, and supporting patients to achieve their personalised healthand care plan objectives and providing interventions such as self-managementeducation and peer support.

Youwill manage a caseload of patients, according to the needs, priorities andsupport required by individuals in the caseload. Your caseload will includepatients with chronic diseases, with pre-diabetic and newly diagnosed Type 2diabetic patients being a particular priority. You will support, motivate, andencourage patients to make positive lifestyle changes, including diet andexercise, to help improve their health and wellbeing. You will also supportinitiatives to improve the health and wellbeing of local NHS staff.

They will have good communication andnegotiation skills and a firm belief that people have untapped resources withinthem, that can be unleashed by providing a high quality, non-judgementalservice. Person Specification

Qualifications

Essential

Be willing to attend training with a non- clinical SSM health coaching skills programme (minimum 4 days) Health related qualifications minimum level 3 Health and Well-being Coaching, nutrition, weight management, personal training, diabetes

Desirable

Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical SSM Health Coaching through a PCI- accredited organisation Personalised Care Institute (PCI) accredited trainer or organisation

Experience

Essential

Experience of using coaching approaches/frameworks and models or other similar helping strategies Motivational Interviewing Experience of working in a multidisciplinary team Experience of working in health and social care care/community development setting or similar

Desirable

Experience of successful partnership working across statutory, voluntary and community sector Experience of working in a primary care setting Networking experience An understanding of the biopsychosocial model of health and the social determinants of health.

Skills & Knowledge

Essential

Understanding how to apply health coaching in group settings Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving Demonstrable skills in supporting behaviour change Excellent group and one-to-one facilitation skills including conflict resolution Skilled in active and reflective listening, building trust and rapport quickly Good people management skills Ability to work with minimal supervision and act decisively and ask for help when needed Proficient in MS Office and web-based services Ability to travel across multiple sites with full UK driving license

Desirable

A good understanding of the evidence base and development of self- management in the UK Understanding of the importance and process of helping people with long- term conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available, such as the Patient Activation Measurement (PAM) Excellent communication and presentation skills Experience with SystmOne and EMIS clinical record systems

Meets DBS reference standards and criminal record checks Ability to work flexible hours and travel out of area as necessary

Proficient speaker of another language to aid communication with people in the community for whom English is a second language
