It focuses on patient-initiated follow-up (PIFU) pathways, personalised care planning, and self-management support. This role presents an exciting opportunity to lead a transformative project that will shape the future of healthcare delivery within our Uro-oncology service at WMS offers comprehensive care for prostate cancer patients, including fast-track diagnosis, a range of treatment options, such as proton beam radiotherapy, and clinical trials for local and tertiary referral pathways.

Main duties of the job

The PCC ANP for Focal will report to the General Manager with additional support from the Urology Matron and Lead CNS. The objectives for the role may change and will be reviewed throughout the year but the main aims of the post are as follows:

o Work closely with clinical team to establish clearly documented and standardised pathways, in particular for post focal treatment and active surveillance patient cohorts as well as ensuring streamline process from point of diagnosiso Ensure adequate safety netting for patients, in particular relating to dischargeo Strengthen nursing input across the prostate cancer team, linking in with other trusts as needed o Support with educating the wider team, including new fellows, creating education materials as neededo Support with patient focused sessions - ensuring their voice can be heardo Ensure Holistic Needs Assessments and discharge summaries are being completed
