Recovery Coordinator

About the role:
As part of a MDT, identify, and engage with, people who have prescription medication dependence problems with complex physical and/ or mental health needs, including co-morbidity. Build therapeutic relationships and deliver a tailor-made package of care including Recognised psycho-social interventions, 1:1 key work, POD and group work. Carry out comprehensive assessments and develop recovery plans that clearly identify how they will enable progression in addressing substance/medication use and re/integrating into the community where applicable. Support the GPs and the MDT in implementing strategies for managing challenging, aggressive, or abusive patients. Refer patients who require support for complex poly-substance use into West Sussex DAWN.About you:
Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field. Excellent knowledge and understanding of prescription drug dependence, related health and co-morbidities, or ability and willingness to learn. Experience of working in a highly pressurised environment and the ability to use strategies to prioritise and manage work-load. Experience of working with clients with complex physical and/ or mental health needs including co-morbidity.What we will give to you:
