Fixed Term 5 monthswith a view to extension subject to national DES contracting.
An excitingopportunity has arisen to join our multi-disciplinary PCN team.
We are a dynamic,forward thinking PCN team who are passionate about developing and deliveringexcellent quality local services to meet the needs of our patients. We are allsmall traditional partner led GP practices and we try and keep the bestfeatures of traditional General Practice while looking to make the most of newopportunities for our patients.
Due to the changesin the ARRS Scheme to introduce General Practitioners we are looking to expandour team by seeking to employ a newly qualified General Practitioner to workalongside our practices, Primary Care Network and Integrated Neighbourhood Teamstaff.
Main duties of the job
To perform as anautonomous general practitioner working independently and with other healthcare professionals to assess, diagnose and treat the conditions of patientsattending within a primary care setting.
Provide expert professional advice topatients, carers and colleagues and ensure the maintenance of clinicalexcellence. To be involved in the provision of education and training to GP ST3sand other staff.
To be professionally accountable and responsible for allelements of clinical practice in accordance with the GMC code of conductensuring the safety of patients and quality of their care is primary.
Providingcounselling and health education
Admitting ordischarging patients to and from the caseload and referring to other careproviders as appropriate
Recording clear andcontemporaneous consultation notes to agreed standards
Collecting data foraudit purposes
Compiling andissuing computer-generated acute and repeat prescriptions
Prescribing inaccordance with the ICB prescribing formulary whenever this is clinicallyappropriate
The post-holderwill be required to visit patients during surgery time in an emergency ifclinically indicated.
The Frailty sessionwill involve working with our care coordinator and social prescribers toprovide pro-active care in the community to frail patients, particularly thosefrom deprived areas who have recently been admitted to hospital or who are atrisk of being admitted

Job responsibilities
In accordance withthe practice rota, as agreed, the post-holder will make him/her-self availableto undertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking andsigning/authorising repeat prescriptions and dealing with queries, paperworkand correspondence in a timely fashion.
Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation
Assessing thehealth care needs of patients with undifferentiated and undiagnosed problems
Screening patientsfor disease risk factors and early signs of illness
In consultationwith patients and in line with current practice disease management protocols,developing care plans for health

The post-holder maybe expected to take part in the Enhanced access rota.
The post-holderwill undertake a fair share of home visiting sessions per week as allocated.There is a local visiting service so it is anticipated that routine visitingwill be minimal. However it is anticipated that planned visits will beundertaken during the Frailty session as part of anticipatory care planning for this group ofpatients.
