About The Role

As a Care Coordinator you will:

· Coordinate and communicate shift rotas for our team of care staff.

· Use effective communication on all rota changes to colleagues and clients.

· Participate in the out of hours on-call service on a rota basis

· Build and maintain positive relationships with our clients, family members, colleagues, and other professionals.

· Utilise the correct systems and technology to enable delivery quality care.

· Support the registered manager with new staff Introductions and inductions.

Uphold the principles of Altogether Care's values.

· Monitor the implementation of safe working practices, following our policies an procedures.

· Be part of a truly caring and processional team that support you as well as we would our clients.
