Job summary

Gillingham Medical Practice are looking to recruit a full time Care Navigator to join the team. Parttime applicants may also be considered.

We are a thriving practice with sixGP Partners, providing high quality primary healthcare to a geographicallyextensive rural population of over 13,000 patients. We are innovative andforward-thinking, and like to work collaboratively with our team and external partnersto provide the best possible care to our patients.

Our team of Care Navigators provide a vital first point ofcontact for our patients. Your role is key to ensuring patients receive thecorrect care within the practice by gathering information for our clinicians.

Main duties of the job

This is a demanding role and we do require a flexible andproactive attitude with the ability to cover for additional hours that willarise during particularly busy periods or to allow for holiday cover.

Excellent customer service and telephone skills and anempathetic approach are vital for this role.

A good working knowledge of IT and proficient keyboardskills are important, as well as the ability to keep calm under pressure.Training will be given on our clinical systems, and we have an established andeffective induction process.

We are a friendly, high performing semi-rural GP Practice inNorth Dorset, within easy reach of the A30 and A303. We have two surgeries: TheBarn Surgery, based near the centre of town and Peacemarsh Surgery just a shortdistance away to the north of the town.

Job description

Job responsibilities

The Care Navigation role is a key position, being first point of call to all patients and visitors. You will be working as part of a small team ensuring that patient experience runs smoothly.

This varied role requires the following skills:

- Advising patients of test results

- Registering new patients

- Processing eConsults

- Photocopying, and filing of records

- Answering all incoming calls from patients daily

- Dealing with any requests from patients face to face

- Issuing repeat prescriptions

- Possess excellent communication skills, both spoken and written

- A good understanding of Microsoft office, to include Word

- Can maintain confidentiality and data security at all times

- Possess some experience within a medical or NHS environment (not essential)

- Can adapt and manage workload dependent on the priority level

- Possess experience within a customer facing role.

Person Specification

Qualifications

Essential

Educated to GCSE level or equivalent Mathematics & English (C or above). Excellent written and spoken communication skills. Some experience within a medical environment(not essential). Punctuality, and excellent customer service skills. The ability to adapt and prioritise according to workload and demand Good IT skills, including Microsoft Office. You must be friendly and approachable, with the ability to work as part of a team and liaise appropriately with other professionals. Also an understanding of the importance of confidentiality and data security. Some flexibility to work shifts from 08:15-1830. Also providing cover for holidays and sickness. This may include 7:00am early morning start and 7:30pm finish. You will be required to work across both our GP sites, The Barn Surgery & Peacmarsh Surgery.

Desirable

Previous experience working in a healthcare setting.
