An exciting opportunity has arisen for a Band 6 Sister/ Charge Nurse/ District Nurse to join our Gorleston Primary Care Home teams delivering high quality nursing care to the population of Great Yarmouth and Waveney. If you are motivated, adaptable, forward thinking and you see yourself leading within community healthcare, then this could be the development opportunity for you. The role contains variety every day with a mixture of face to face clinical practice, triage, leadership and management. The role includes delivery of planned and urgent community response visits as well as supporting patients under the Step Up Virtual Ward. You will be responsible for the line management of a number of clinicians, have the opportunity to participate in HR processes including recruitment, appraisals and supporting junior members of the team. This role sits in a multidisciplinary team providing community services 7 days per week 07:00- 20:30 hours. The role requires
weekend and bank holiday working on a rotational basis.
The post is offered up to hours per week, part time hours are available.
Following the announcement of a Agenda for Change pay award, this role will be eligible for a salary increase effective in October 2024 pay. Details of the new pay structure can be found on the NHS Employers website.
Main duties of the job
To provide clinical leadership, operational management and professional support to community nursing within Primary Care home.
To work with other members of the multidisciplinary team to provide planned and unplanned care.
To provide expert clinical case management for patients living in the community who require nursing care.
To work autonomously at a level appropriate to clinical competence and within scope of
professional practice
To carry out rapid assessments in patients own home with the aim of preventing admission to the acute hospital. These assessments will take a holistic ethos including physical, physiological and social aspects of need.
To assess and monitor patients admitted to the virtual ward alongside colleagues from Norfolk Community Health & Care.
Support the delivery of the Urgent Commuity Response including outside of core hours.
Support delivery of the triage function of the PCH's ensuring moist effective use of clinical time and best outcomes for patients.

Primary Care Homes are a multidisciplinary clinical team which include other Nurses, Occupational Therapists, Physiotherapist, Assistant Practitioners, Registered Nurse Associates, Rehab Support Workers, Healthcare Assistants and Phlebotomists. The team work in partnership with Primary Care, Social Services, social care providers, other specialist community health teams and voluntary sector providers to ensure patients are able to remain in their own homes whenever possible.

Clinical
To be professionally and legally responsible and accountable for all aspects of own work, including the management of patients in your care.
To undertake the comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills.
To accept clinical responsibility for a designated caseload of patients, and to organize this efficiently and effectively with regard to clinical priorities and use of time.
To ensure a high standard of clinical care for the patients under your management, and support more junior staff to do likewise.
To formulate an individualised management and/or treatment plan, using clinical reasoning skills and utilizing a wide range of treatment skills in order to formulate a specialised programme of care. This will ensure patients meet their re-ablement goals and promote independence.
To assesscapacity, gain valid informed consent and have the ability to work within alegal framework with patients who lack this capacity to consent to treatment.
To assessfor environmental aids and arrange for provision or provide advice on whereaids can be obtained.
To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of the service and to ensure a consistent approach to patient care.
Tocommunicate effectively and work collaboratively with medical, nursing andtherapy colleagues to ensure delivery of a coordinated multidisciplinaryservice. This will include discussion ofpatient care and case management.
To communicate effectively with patients and carersto maximize re-ablement potential and toensure understanding of their condition.
Managerial
Be the named nurse or deputise forthe Primary Care Home named nurse for an identified Primary Care practice andcreate relationship with the Lead GP, Practice Manager and Lead Nurse as arepresentative and of the wider Primary Care Home team.
Training,supervision and performance management of other members of themultidisciplinary team which may include Assistant Practitioner ,NursingAssociates, Therapy Assistant Practitioners, Health Care Assistants and RehabSupport Workers and students, with assistance from more senior members of theteam. This will include the use offormal appraisal documentation.
To provideteaching/training to other members of the Multi-Disciplinary Team.
To comply with the organisational and departmental policies and procedures and to be involved in the reviewing and updating as appropriate.
Be actively involved in the collection of appropriate data and statistics for the use of the service.
Todeputise for Band 7 in terms of operational issues when appropriate.
Participate in any other duties which may be deemed appropriate by the management team.

Be aware of the Health and Safety aspects of your work and implement any policies which may be required to improve the safety of your work area, including the prompt recording and reporting of accidents to senior staff, and ensuring that equipment is safe.

Experience
Essential
Experience working within a similar field of nursing/healthcare Experience of working as part of a multi-disciplinary team Experience of working with frail older adults and awareness of the complex issues relating to this client group.
Desirable
Previous experience at band 6 level. Clinical experience gained in a community/intermediate care setting. Student supervision/clinical education. Experience of clinical audit. Experience of successful change management. Experience of facilitating clinical supervision.
Qualifications
Essential
Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult) BSc in Nursing Practice or equivalent. Evidence of continued professional development and a professional portfolio. Leadership/ management qualification or willing to undertake. Health Coaching programme.
Desirable
Specialist Practitioner Qualification in District Nursing. Community Nurse Prescriber v100 /v150. Independent non- medical prescriber.
