Job summary

The West Suffolk Nephrology team is currently looking for a Fixed Term Consultant Nephrologist to complement the current team, as we expand and look forward towards the new hospital. The applicant would be looking after patients on the Nephrology ward which is currently shared by Dr Petchey, Dr Karimi and a General Physician. The consultants are currently rotating on and off the ward. This provides opportunity for service development and allowing the applicant to have dedicated time to develop their sub-speciality interest.

We would hope that the applicant would be able to continue the practical skills of renal biopsy at inpatient level. The hospital is also looking towards developing acute dialysis as we move towards the new hospital which will require a separate on-call rota.

Main duties of the job

The applicant will be looking after inpatient (mixed with general internal medicine and Nephrology) and will be part of the General Medical on-call Rota.

Current clinics would be General Nephrology patients (Chronic Kidney Disease and glomerulonephritis). West Suffolk Hospital has very close ties with Cambridge University Hospital and all of the current Nephrologists have Honorary Consultant Contracts, as we provide consultant cover for the satellite dialysis unit, Vasculitis Clinic and Transplant Clinic.

As West Suffolk Hospital supports Cambridge University in their post graduate medical programme - we could be looking for applicants who would be interested in teaching.

Nephrology

Current nephrology services at the WSH include inpatient management of renal patients on ward F8 - encompassing patients with acute kidney injury, management of patients with CKD and review of dialysis and transplant patients. There is a strong emphasis on early detection and management of patients with Acute Kidney Injury (AKI) and overseeing referrals of such patients form a significant proportion of the workload. There is a joint nephrology-critical care ward round of ITU patients weekly.

Outpatient services include weekly general nephrology clinics, a transplant clinic, a low clearance clinic, monthly diabetes/renal clinic held jointly with Dr Marath (Acute Medicine/Diabetes) and a paediatric transition clinic held with Dr Christian on a 6 monthly basis. There is a monthly cardio-renal MDT, and Nephrology support for the Medical Obstetrics clinic is provided on an ad hoc basis. Tertiary level nephrology care is provided at CUH including acute transplantation and vasculitis.

Currently the Dialysis Unit is supervised jointly by Dr Chong and Dr Karimi. The Nephrology service is currently expanding with Dr Yiu leading the way in Virtual Ward for Acute Kidney Injury and Nephrotic Syndrome. We have clinical nurse specialist whose main focus is the transplant and predialysis service. There is an expectation that enhanced renal diagnostics and vascular access procedures will be repatriated from Addenbrookes Hospital to the Trust in the next 12 - 24 months.

Medicine

In-patients are triaged to their most appropriate medical speciality ward cardiac, acute respiratory, stroke unit etc. There is a well-structured multidisciplinary team meeting on each ward which will set goals to facilitate a reduction in length of stay.

Person Specification

Education, Qualifications and Special Training

Essential

MRCP or equivalent Full GMC registration

Desirable

MD or PhD MRCP (Nephrology) Entry on the GMC specialist register in Nephrology and General (Internal) Medicine or be eligible for entry (within six months of attaining Certificate of Completion of Specialist Training) at the time of interview

Experince

Essential

Experience in management of acute medical inpatients and medical outpatients Ability to take full and independent responsibility for clinical care of patients Experience in management of Nephrology in-patient and out-patient medicine Ability to offer expert clinical opinion on a full spectrum of Renal patients including patients with suspected or proven vasculitis

Desirable

Five years supervised training in an appropriate SpR training programme or equivalent Experience of managing vasculitis inpatients and clinics in Tertiary centre, dedicated Fellowship programme, or similar Clinical training and experience equivalent to that required for gaining UK CCT in General Medicine Clinical training and experience equivalent to that required for gaining UK CCT in Nephrology

Management and Audit

Ability to develop and manage a multi-professional vasculitis service encompassing inpatient reviews and outpatient clinics Ability to organise and manage day to day service delivery out-patient priorities Ability to manage and lead strategic developments in service delivery Experience of audit management Ability and willingness to work within the Trust and NHS performance framework and targets
