Job summary

After a planned expansion of our list size in 2023 we areseeking to appoint Practice Nurses, with opportunities available for both fulland part time employees.

The successful applicant will join a well-established team ofdedicated and friendly multidisciplinary staff which includes 8 NursePractitioners.

The position would be suitable for applicants with a registerednursing qualification seeking to develop skills in autonomous clinical work.

James Alexander Family Practice is an innovative forward thinking practice, which holdsit staff and patients at its heart, treating all staff like family.

Main duties of the job

Working alongside the rest of our nursing team you will beresponsible for delivering practice nursing services to our patient populationincluding monitoring of long-term conditions, health promotion and screeningactivities.

You will work collaboratively with our extremely supportivemulti-disciplinary team to meet the needs of patients and provide nursing leadershipas required.

We have 3 salaried GPs and 8 Nurse Practitioners, all ofwhom are prescribers.

We have a strong nursing team with 2 Practice Nurses and 3 Health Care Assistants.

Early in 2016 we went through extensive organisationalchanges to help us future proof our organisation and create efficiency savingsand streamline services. This helped strengthen our non-clinical team, with ourcurrent non-clinical team working effectively providing high quality care andsupport to all our clinical staff and patients.

We have also complimented our strong clinical team with adedicated Pharmacy team.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. Theremay be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:

Develop, implement and embed health promotion and wellbeingprogrammes

Implement and evaluate individual treatment plans for chronic diseasepatients

Identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects on the patients health

Provide routine nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF

Provide wound care to patients

Provide contraception advice

Perform cervical screening

Provide travel medicine services

Request pathology services as necessary

Process pathology results as required

Provide chronic disease clinics, delivering patient care as necessary,referring patients to secondary / specialist care as required

Maintain accurate clinical records in accordance with currentlegislation

Ensure read codes are used effectively

Maintain chronic disease registers

Chaperone patients where necessary

Prioritise health issues and intervene appropriately

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Implement vaccination programmes for adults and children

Support patients in the use of their prescribed medicines or over thecounter medicines (within own scope of practice)

Liaise with external services / agencies to ensure the patient issupported appropriately (vulnerable patients etc.)

Delegate clinical responsibilities appropriately (ensuring safe practiceand the task is within the scope of practice of the individual)

Support the clinical team with all safeguarding matters, in accordancewith local and national policies

Deliver opportunistic health promotion where appropriate.
