Excitingopportunity for an Advanced Nurse Practitioner (prescriber)

We are looking for someone who enjoys providingperson centred care, is happy to work independently and seek advice whennecessary. The successful candidate will receive ongoing mentorship and support.

This is a part-time position up to a maximum of hours a week, and flexibility on working hours will be considered.

Main duties of the job

The role will include seeing urgent / same day appointments as part of the multidisciplinary Duty team, supported by a dedicated Duty GP. You will also be required to provide home visits and support care home visits, when necessary.

We are looking to recruit a clinically strong andhighly motivated Advanced Nurse Practitioner (prescriber) to join our fantasticmultidisciplinary team consisting of advanced health professionals including,Advanced Nurse Practitioners, Clinical Pharmacists, MSK specialist, AdvancedParamedic Practitioners, Practice Nurses, Nursing Associate, HCAs, PharmacyTechnicians, dedicated Frailty Team and Social Prescribers along with salariedGPs and Partners.

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Develop, implement and embed health promotion and wellbeing programmes Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice Implement and evaluate individual specialised treatment plans for chronic disease patients Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Undertake the collection of pathological specimens Request pathology services as necessary Process and interpret pathology and other test results as required Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required Maintain accurate clinical records in conjunction with extant legislation Ensure SNOMED CT codes are used effectively Chaperone patients where necessary Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Be an extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required Contribute to practice targets (QOF, etc.), complying with local and regional guidance Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.) Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual) Support the clinical team with all safeguarding matters in accordance with local and national policies Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Deliver opportunistic health promotion where appropriate

Secondary Responsibilities

In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:

Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary Monitor and ensure the safe storage, rotation and disposal of medicaments Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the organisation Develop an area of specialist interest, taking the lead within the organisation Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach , root cause analysis (RCA) Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation Person Specification

Experience

Essential

Experience of prescribing and undertaking medication reviews Experience of working in a primary care environment Registered Advanced Nurse Practitioner with the NMC (Nursing and Midwifery Council) Independent/extended Nurse/Paramedic Prescribing qualification. As an independent prescriber, you will be responsible for safely prescribing medication to complement treatment plans for a range of medical conditions.

Registered Nurse with Nursing and Midwifery Council Masters degree required for qualification post December 2020 Post graduate diploma or degree for Advanced Practice Qualification up to December 2020 Qualified Independent Nurse Prescriber on the NMC register Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet Meets the standards for registered ANP working at advanced level Minor illness qualification

Personal Qualities

Essential

Background of working in a fast-paced Primary Health Care or acute environments is essential. Experience in dealing with a wide range of minor ailments. Expected to practice autonomously, carrying out in-depth clinical assessment of patients, formulating clinical decisions and treatment pans within local guidelines/policy to manage multiple complex healthcare needs both acute and chronic. Ability to manage conflicting work priorities in a high-pressure environment. Excellent communication skills with ability to deal effectively with both patients and members of staff. Excellent IT skills. Excellent organisational and prioritisation skills. Participate in the development of services and assists the implementation of change. Can do and positive attitude. Hold a valid UK Drivers License.
