Job overview

The older adult community mental health team is excited to be recruiting 2 further Band 5 newly qualified mental health nurses to join our busy but rewarding community team.

The successful candidates will join a dedicated staff group providing assessment and evidence-based treatment interventions to a wide range people in South Bucks and the surrounding area, including the parish town of Marlow, market Town of Amersham and High Wycombe with surrounding rolling country side of the Chiltern hills. 

The team is part of the Transformation programme for mental health services with a focus on treatment and we have a number of Health and well being practitioners and community support workers. 

We have incorporated the Community Mental Health Framework into the daily functioning of the team; which enables us to be focused on the recovery of the patient through a multidisciplinary (MDT) approach; with expertise from doctors, social workers, nurses, occupational therapists, support workers and psychological therapies staff.

We also have twice weekly MDT meetings, where we discuss clinical matters within our supportive team. We strive to provide person centred evidence based care to patients, where possible working collaboratively with their families and carers. 

Main duties of the job

This role is open to Newly Qualified Mental Health Nurses and will be supported by the Trust preceptorship programme.

You will be expected to deliver and maintain high quality and meaningful care for our patients. You will need to be a highly motivated practitioner who has experience of mental health services and can demonstrate the ability to initiate and manage your work effectively. The team is highly supportive and committed to delivering high quality patient care; where the caseload can be demanding, diverse and frequently complex.

You will need to be able to work autonomously, with a caseload of patients with whom you will engage and offer the recommended treatment and support. You will have experience of case management and be able to assess and plan treatment with service users. You will also be able to assess and manage risk, including the use of Safeguarding processes.

You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and hold your development as central. We offer regular supervision and you will be encouraged and supported to undertake any training relevant to your professional development.

Detailed job description and main responsibilities

We are a supportive, dynamic and integrated team which although busy has many rewards. We care deeply about our patients and their families and carers. The South Bucks Older Adult Mental Health Team's main base is at Saffron House. The team is split into three Consultant Lead teams; which are aligned to local Primary Care Network patches. 

The South Bucks older adults team works with non working age (65 and above) who experience a wide range of mental health conditions. We are committed to the person-centred recovery approach in all our clinical work and pride ourselves in offering a high-quality service reflected in our CQC rating of Good. 

The South Bucks older adults is a wonderful team to work in and this role offers an opportunity to develop senior clinical and leadership skills. The Management team are committed to the wellbeing and development of the staff working in the team. 

Under the guidance of a named supervisor, take day-to-day responsibility as Lead Professional or Key Worker under the Community Mental health Framework for an identified caseload of clients with needs of moderate complexity.
