Job summary

An exciting opportunity has arisen for a practice teacher/clinical team leader to join our Intermediate Care Team.

The post holder will be welcomed into a substantial and supportive rehabilitation team. We are looking for a highly motivated and passionate band 7 nurse who can provide clinical expertise in the delivery of nursing care and leadership within the team.

Main duties of the job

To support the Operations Manager in delivery of this wonderful service and to adopt a supportive culture for teamwork and building. The Intermediate Care Team cares for patients needing rehabilitative/social care within Community Rehabilitation Units throughout Hull and East Riding and through our UCR 2-Day service seeing patients within their own home and preventing hospital admission.

Previous rehabilitation experience would be an advantage.

Qualifications

Essential

RGN/RN/Diploma Nursing Studies Post graduate Practice Teacher Nurse Prescriber V100 Evidence of ongoing continuing professional development/Portfolio

Knowledge

Essential

Current Legislation and impact on community nursing services. Demonstrate clinical evidence based practice Mentorship qualification and evidence of supervision/teaching/team management Mentoring of post graduate nursing students studying for the BSC/specialist practitioner To demonstrate an understanding of proactive caseload management. Understanding of audit processes, Key Performance Indicators and Targets, CQUIN collection data, Quality Monitoring Programmes Integrated Team working and partnership working, with an MDT approach to patient centered care To be aware and be proficient in budgetary management

To have an understanding of the principles of restorative practice

Experience

Essential

Extensive Post registration experience at band 6 level Experience in planning and providing care to patient with Long Term Conditions, wound management/Tissue Viability, palliative and end of life care. Experience as an Autonomous Practitioner Developing and leading practitioners to deliver client responsive services. Experience of proactive caseload management Experience of collaborative working across organisation both statutory and voluntary services Supervising team members and team management. Experience of initiating and implementing clinical or professional change and evaluation of practice Experience in using IT in clinical practice Risk assessment completion, and Health and Safety Awareness Involvement with project work and presentation skills
