The School Nursing team deliver a service that is commissioned by Solihull Metropolitan Borough Council and provided by South Warwickshire University NHS Foundation Trust, which is ranked in the top 5 in the staff survey and rated outstanding by CQC.
Main duties of the job
The role requires a solution focused team player, delivering an evidence-based service, using a range of assessment and intervention tools to deliver the Health Child Programme, whilst managing a caseload including safeguarding and working collaboratively with a wider multi-disciplinary 0-19 service and local voluntary sector, ensuring that key performance indicators are delivered..

Job responsibilities
Oversee the delivery of The Health Questionnaires: parents, Year 6 and Year 9.
Respond to Health Questionnaire alerts within timescales; make professional judgement to assign/delegate care to skill mix team. Ensuring you evaluate care provided by receiving feedback from the team on the outcomes of interventions.
Offer brief intervention to ensure timely response to identified need, and discharge appropriately.
Have an understanding of the health needs of the local geographical area by means of using profiling tools/accessing local data information from reliable sources, chimat, public health observatory to inform work undertaken and future practice development.
To develop networks with Education, Social Care and other statutory and voluntary agencies, to work collaboratively in achieving National and local priorities Acting Early initiative.
Lead at least 3 health public health initiatives a year which are consistent with nation/local messages, sexual health, risk taking behaviour or emotional needs.
Actively contribute to appropriate planning and management meetings within the school environment to ensure the health care of children is being adequately addressed.
Working in conjunction with Health Visitor colleagues support to transition into Primary School of those children with identified needs.
Identify and support children, young people and families in need of universal plus services through the Common Assessment Framework process. This includes taking the role of Lead Professional where required and appropriate.
Work with parents, carers, specialist health professionals and others to develop and review in-school care plans for pupils with complex health needs and long term conditions annually or sooner if required. Including providing or co-ordinating support, education and training for families, carers and school staff.
Act on/document if required, intelligence or surveillance received from other sources, A&E forms, domestic abuse referrals, priority families, excluded children.
Have knowledge and understanding of Solihull Safeguarding Children Board policies and procedures. This includes initiating referrals to Childrens services when a child or young person is at risk. Providing written reports, chronologies as required attending initial case conferences taking part in risk assessment discussions and agreeing a plan for the child. Being part of review conferences and core groups where health needs are identified and planned work with the child or family is required.
Promote the school health service positively to other agencies and professionals.
Deliver work that is both researched and evidence based, follow and adhere to the standards and guidelines set for School Nursing to ensure a consistent approach is followed.
Submit accurate and timely activity and outcome datato ensure that key performance indicator targets set by the Trust/and or the local authority are reached.
Support children, young people and families during transition into school, and from primary to secondary and secondary to further education/ work.

Promote the uptake of the childhood immunisation programme when opportunities arise with contacts with children, young people and families.

Promote a whole school approach to obesity; proactively engage with 100% of pupils who have been identified as being obese (95 percentile) refer on to Be Active Be Healthy and other services as required and 100% of pupils who have been identified as being clinically underweight.
Lead on one of the key performance indicator targets set by the Trust/and or the local authority are reached for school nursing services.
Participate in clinical audits and clinical bench marking to improve service care and delivery.
Communications and Working Relationships
Develop and maintain effective communication networks, with service users, health care professionals and other statutory and voluntary organisations especially when communicating highly sensitive information relating to a child and their family.
Raise awareness of health needs and influence policies affecting health.
Raise awareness of health needs within families, using skills of negotiation and motivational interviewing to facilitate change leading to a healthier lifestyle.
Use powers of negotiation when communicating in highly complex and potentially hostile environments for example in the child protection arena.
Organise and complete local induction/training programmes for new staff, and other appropriate workers.
Support the clinical lead in the delivery of the service, ensuring and maintaining high standards of evidence based practice within the city.
Promote positive team working by leading weekly base meetings. This involves organising and delegating work appropriately to the skill mix team, ensuring that work set and identified is supported and completed.
Supervise, mentor and assess nursing students where required and Post registration students undertaking community experience.
Support other team members to develop and apply their knowledge and skills.
Undertake appraisals for the skill mix team to promote staff development.
Support new staff in preceptorship by being a mentor and positive role model.
Promote the school health team positively to other agencies and professionals.
Attend Trust statutory and mandatory training programme specific to the role and ensuring training is completed as per Essential Skills Policy.
