Job summary
Full Time - hours per week. Thepost holder will be expected to work as part of a larger substance misusemultiagency team, supporting on site dispensing of methadone, oral lyophilisate(Espranor) and subcutaneous buprenorphine (Buvidal).
The post holder should enjoy face-to-face contact with people who haverecognised that their use of substances has become problematic. They will have excellentcommunication skills, be able to work as partof a multidisciplinary team, across agency cultures including those withinstatutory, private and voluntary sector partnerships. The post holder must be flexible andefficient within small locality teams, to ensure services aredelivered equitably and with the service user atthe heart of delivery.
Main duties of the job
Clinical services include substitute prescribing for opiate dependence, harm reduction initiatives including the provision of take home Naloxone; family planning and sexual health screening; Blood Borne Virus testing and referral on to specialist support.

Job responsibilities
The Substance Misuse Nurse will:
• Deliver onsite dispensing and supervision of substituteopiate medication.
• Follow Controlled Drug Policies and Procedures.
• Provide advice, information, and guidance on themanagement of substance misuse to service users engaged with services.
• Undertake comprehensive clinical assessments
• Agree treatment plans with service users, and reviewthem regularly
• Undertake drug screening (urine, fingerprint and oral).
• Ensure case notes and other statutory records are up todate and kept in accordance with Kaleidoscope policies and procedures /regional information sharing protocols
• Deliver clinical interventions, including HCV testing,HBV immunisation and naloxone training/administration where appropriate and inpartnership with Local Health Board and PHW colleagues.
• Deliver subcutaneous injection of buprenorphine(Buvidal).
• Liaise with community pharmacies to ensure prescriptionsare in place and monitored appropriately. Community Substance Misuse Nurse -South Wales
• Provide advice, support and training to GPs,pharmacists, and other professionals where appropriate.
• Participate in clinical audits and service reviews.
• Facilitate the transfer of service users from secondaryto primary care services, and lead with referrals / liaison with CommunityMental Health Teams (CMHT’s) and other community substance misuse treatmentproviders on leaving Kaleidoscope services.
• Actively encourage service user involvement andfeedback and ensure views are considered in informing the development of theservice.
• Collate and enter data electronically and contribute toservice reports.

Experience
Essential
-Excellent care co-ordination skills, with experience in undertaking comprehensive clinical assessment, care-planning and treatment reviews with people who have diverse and complex needs. -Experience and knowledge of working with substance misusers. -Experience of using electronic case management / clinical notes recording system. PalBase / PARIS/ DELIUS
Desirable
-Experience of treating individuals requiring alcohol detoxification. -Experience of developing and maintaining successful partnerships with GPs, treatment agencies and other healthcare providers. -Experience in undertaking clinical audits. -Experience of delivering outreach and engagement model of treatment and care. -Knowledge of housing options pathways and experience of working with individuals at risk of homelessness.

Essential
RGN/RMN with valid NMC Pin.
Desirable
-One year post registration experience in Substance Misuse -Independent/supplementary nurse prescribing qualification -Training in Anaphylaxis and BBV -Relevant training in Phlebotomy -Relevant specialist qualification in alcohol screening and brief advice and substance misuse treatment.
