Job summary
We are looking torecruit an experienced and enthusiastic prescribing pharmacist. This role willbe practice based, but you will work closely alongside the rest of the pharmacyteam, which consists of a practice based pharmacy technician and two PCNclinical pharmacists.
The successful applicant will play a lead role in medicines management,optimising prescribing and improving quality. The post holder will undertakemedication reviews to proactively manage patients with complex polypharmacy andwill implement prescribing decisions and request appropriate monitoringinvestigations.
The post holderwill work closely to support the wider practice teams to support safe andeffective prescribing.
We are looking for someone to work between 26 and hours. Please make it clear in your application the hours that you are looking for.
Main duties of the job
We are seeking a prescribing pharmacist to work with our clinicians andpatients to carry out medication reviews, practice as an independent prescriberworking within the scope of their practice and carry out face to face andtelephone consultations with patients as required. The ideal candidate will beable to encourage cost effective prescribing within the practice, ensuremedications are prescribed and monitored safely and assist with QOF and IIFtargets, and other patient care incentives.

Overdale Medical Practice is a well established surgery covering anumber of villages on the Nottingham / Derby border. We have over 11,500patients and the successful applicant will be supported by a multi-disciplinaryteam of nine GPs, three advanced nurse practitioners, two long term conditionnurses and three other practice nurses. Through our PCN we also have a pharmacy team, firstcontact physio, social prescribing team and mental health practitioner. We benefit from ahighly experienced administrative team and reception team. We aim to providethe highest quality care to our practice population. We receive excellentpatient feedback, which has us ranked within the top 4% of practices in thecountry.
Job description
Job responsibilities
Carryout structured medication reviews on patients with complex polypharmacy,implementing prescribing decisions and requesting appropriate tests
Reconcilemedicines following hospital discharge and work with patients and communitypharmacists to ensure patients receive the medicines they need post discharge.
Reconcilemedicines from clinic letters, with support from the pharmacy technician
Supportlong term condition reviews
Carehome ward rounds to carry out medication reviews and deprescribe whereappropriate
Contributeto reductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines related issues
Interfacewith community and hospital pharmacy colleagues
Managepatients and health care professionals medicine queries
Increasequality and safety of prescribing through mechanisms such as audit and PDSAcycles
Contributeto service improvement through QOF and enhanced services
Implementdrug withdrawals and alerts MHRA aimed at improving medicines safety
Workwith primary care professional and patients to implement NICE and otherevidence based guidelines
Contributeto multi-morbidity reviews and multidisciplinary reviews
Providemedicine information and training to Practice healthcare professionals and adminstaff
Act as a source of medicines information forall of the practice team and patients ( around doses, side effects, adverseevents, possible alternatives around out of stocks)
Prescribingadvice to prescribers in practice temporary non availability of drugs
Ensurepatient safety when they are transferred between care providers throughreconciliation of prescribed medicines
Person Specification
Qualifications
Essential
Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council Prescribing qualification Has completed, or is planning on completing, the CPPE pathway
Desirable
May hold or be working towards a postgraduate pharmacy qualification
Experience
Essential
Minimum of 3 years post graduate experience in pharmacy, as demonstrated within a practice portfolio Has an awareness of the breadth of common acute and long term conditions that are likely to be seen in a general medical practice
