Therapists will be employed by Cognus Limited and be a partof the Cognus Therapies Team who are delivering the provision for specialeducational needs within the London Borough of Sutton.

Cognus Therapies is a thrivingmultidisciplinary team of Occupational Therapists, Speech and LanguageTherapists, and Therapy Assistants who support children and young people inschools, colleges, bases and specialist provisions within the London Borough ofSutton and surrounding boroughs.

Job responsibilities

JOBSUMMARY:

Working within a multidisciplinary team of speechtherapists and occupational therapists, the post holder will have theopportunity to work in local schools and colleges across the 4 years to 25years age range or with a smaller age range within this. Advising and supporting local schools / settingswith children and young people with Speech, Language & Communication Needs (SLCN)including a possible cohort with ASD or PMLD and SLCN; to manage their caseload toensure that intended outcomes are delivered within agreed service standards. Raisingawareness of the needs of and strategies available for children and youngpeople with SLCN.

PRINCIPALACCOUNTABILITIES:

To personallyundertake a case load of referrals to include:

Visiting and assessing children and youngpeople with SLCN. To appropriately identify service provision inline with child and young persons needs at specialist, targeted or universallevel of support. Delivery of specialist programmes on anindividual and/or small group basis. Offering advice to parents and carers as wellas teaching staff at a targeted or universal level. Maintaining childrens / young peoplesrecords to a high standard.

SpecificDuties:

To advise andsupport school staff on the education of children and young people with SLCNincluding: Screening andprioritising new referrals in order to determine the need and level of therapyprovision required in line with service criteria. At the point of referral, gathering relevantinformation regarding previous and current input from health and social careservices. Carrying out a range of non-standardised andstandardised assessments and interpreting a range of factors relating to thechild or young persons condition; focusing on access to education. Providing reports to referrers and other agenciesinvolved in the child/young persons care and advice on appropriatedifferentiation of and access to the curriculum. Plan, create and deliver a wide rangeof tailored interventions and/ or therapy programmes where direct SaLT input isindicated and effectively hand over prescribed programmes through training andmodelling with teaching staff, parents and carers. To maintaineffective liaison with all partners parents/ carers, professionals inEducation, Health, Social Care and other Services with particular regard toSafeguarding and best practice To monitor and record child oryoung persons progress and effectiveness ofintervention through goal setting, outcome measurement and evaluation ofpractice and to communicate these with key stakeholders. Collaborationwith a range of partner agencies to create effective transition support forspecific children and young people. Promotion ofeffective inclusion of children and young people into all aspects of the schoolcurriculum. To attend andcontribute to team meetings as required.

Training andDevelopment Responsibility

To be aware ofchanging trends in treatment and promote Evidence Based Practice; embeddingthis into own specialist area of work in order to provide the highest standardof care to service users. Embeddinglegislative change and key developments relating to improved outcomes forchildren and young people with SLCN. To participatein clinical audit, service development activities and research programmes asrequired. To engage withcontinual professional development as identified through managerial andclinical supervision and appraisal, including attending training and courses asagreed. To comply withthe Cognus Limited mandatory training requirements. As part of thetherapies team to plan, deliver and evaluate training sessions for schools andeducational providers. - Educated to degree level or equivalent. - Relevant qualifications including registration with HCPC
