An exciting opportunity has arisen for a highly motivated and experienced Band 5 children’s nurse, passionate about providing the best care to children and young people and their families within the community setting, to join our friendly service at Epsom and St Helier University Hospitals NHS Trust, based at Epsom Hospital.

You will work as part of a service providing specialist nursing care to a group of patients with support and supervision from Senior Children's Community Nurses.

Main duties of the job

Under the supervison of Senior Nurses within the Children's Community Nursing Service:

Be responsible for on-going assessment of acute and complex care needs, the development, implementation and evaluation of programmes of specialised nursing care for sick infants, children, young people and their families/carers in the community.

Provide specialist knowledge to meet the diverse range of needs of patients and their families/carers in a variety of community and hospital settings, acting as a resource of paediatric expertise and provision of specialist advice to other professionals, promoting evidence based practice.

Using clinical knowledge and skills provide care for children and young people with a diverse range of needs including IV therapy, care of central venous access devices, enteral feeding, tracheostomy care, long term ventilation and oxygen therapy.

Support parents and families to care for their child at home, teaching and enabling them to gain the skills required.

Work with all CCNS members as well as the multi-disciplinary team.

Manage own time and a supervised caseload.
