Job summary

Central and North West Foundation Trust - Milton Keynes community nursing - Tissue Viability Service has a new and exciting opportunity to improve the skill mix within our team. We are offering the opportunity for a registered adult nurse to join our team as a band 5 tissue viability nurse.

The successful candidate will be expected to act as an autonomous clinician within agreed protocols, guided by organisational policies and within scope of professional practice.

We are looking for an experienced and motivated nurse, working part time (30 hours) to support the Tissue Viability Nurse Specialists in their role promoting high quality, evidence based wound care advice and education to patients and staff across Milton Keynes health economy . You will be responsible for supporting the community teams, community inpatient units, residential homes and other primary health care staff with the management of patients with complex and or chronic wounds, including pressure ulcers, leg ulcers and other wounds. This will be achieved with the shared aim of promoting excellent care at the heart of the community.

As a Tissue Viability Nurse you will assist with the development and participate in the delivery of the tissue viability educational programme to health care personnel involved with wound care and skin care within the local health economy. This will include both informal and formal group training.

Main duties of the job

To provide support to achieve on targets (CQUIN), Patient Safety Federation etc) to reduce preventable and newly acquired pressure ulcers year on year Contribute to the Pressure Ulcer Ambition within MK Provision of a specialist wound management service to patients with compromised tissue viability including leg ulcers, pressure ulcers and complex wounds. Participation in complex wound clinics across Milton Keynes Pressure ulcer prevalence and incidence monitoring including contributing to pressure ulcer investigations using PSIRF framework. Datix management and reporting across the health economy. Tissue viability expertise, support and training across the health economy including community services, care homes, prisons, all CNWL inpatient facilities in Diggory division and GP practice. Assist with management of pressure ulcer prevention equipment and providing specialist advice and support to prescribers. Risk assessment in relation to pressure ulcer prevention and chronic wound management Implementation and promotion of wound formulary across the health economy. You will report to the Band 7 Tissue Viability Nurse Specialist nurse team lead and work as part of the wider tissue viability team on Trust wide tissue viability related projects.

Job responsibilities

Clinical Duties:-Provision of evidenced based assessment, management and treatment of complex tissue viability cases across the health economy, this could be in a clinic setting, inpatient facility, care home, prison or patients own home.-Documentation of clinical care plans, advice and activity to a high standard.-Liaison with the multidisciplinary team.-Prescription of dressing treatments working within prescribed formulary.-Develop specialist skills to implement tissue viability treatment programmes including advanced wound therapies and debridement.-Assist in complex wound clinics this will include developing skills to carry out holistic lower limb assessments and compression bandaging skills and advanced bandaging techniques.-Provide specialist advice on pressure reducing equipment-Training and education, across the health economy formal and informal. Including mentorship of peers. -Contribute to Management of clinical incidents relating to and contribution to investigations related to this.- Assist with Audit.-Willing to undertake further education to enhance knowledge and skills.Professional:

-Maintain current registration with the NMC-Follow code of professional conduct- Participates in annual development reviews-Participates in training of health care professionals-Attends mandatory, in-service and other training programmes and professional meetings as required-Allocates time for professional development to maintain knowledge and skills-Contributes to rolling programmes of education and training-Teaching informally in the clinical environment-Identifies clinical risk and reports on incidents and is involved in investigations. -Undertakes systematic caseload and workload review-Provision of data and evidence of activity as required
