Main duties of the job

We are offering a rewarding opportunity for a clinicalpharmacist to become an integral part of our team in our practice. In this role, yourprimary responsibilities will involve conducting medication reviews, managingchronic disease, responding to drug information queries, promoting adherence totreatment plans and educating patients on medication use and potential sideeffects.

Job responsibilities

Work as part of amulti-disciplinary team in a patient facing role to clinically assess and treatpatients using expert knowledge of medicines for specific disease areas. Providepharmaceutical consultations to patients with long term conditions as anintegral part of the multidisciplinary team. Provide proactiveleadership on medicines and prescribing systems to the practice team, patientsand their carers. Improve patient andcarer understanding of, confidence in and compliance with their medication. Provide advice andanswer medication related queries from patients and staff Improve the qualityand effectiveness of prescribing through clinical audit and education toimprove performance against NICE standards, and clinical and prescribingguidance. Contribute to apatient safety culture through reporting and investigation of medicinesincidents and undertaking proactive measures to improve patient safety. Work with communitypharmacists, hospital pharmacists, and other stakeholders in the medicinessupply chain to improve patient experience and manage incidents. Respond toprescription requests from community patients, nursing/care home staff,pharmacy staff and GPs, processing them within recognised timescales ( 48hours for routine prescriptions and same day for urgent prescriptions). Thiscan include re-routing prescriptions to out of hours pharmacies for same daydelivery. Use practicetemplates to record near patient monitoring for high risk drugs. Identify bloodtests, recalls and reviews using established protocols and forward these to theappropriate clinician. Support and respondto medicines-related queries from medical and nursing colleagues and frompatients and relatives via face to face, via phone or via email. Queries caninclude issues with interactions or ADRs, availability of stock, swallowingdifficulties, cost-effectiveness, urgent requests or extra supplies. Review hospitaldischarge notifications and outpatient hospital letters and reconcilemedication against current repeats, flagging up and correcting discrepancies. Promote safe use ofmedication, reporting of medicines-related incidents and pro-activelypreventing safeguarding incidents. Use knowledge of GPcomputer prescribing systems and community pharmacy dispensing systems toadvise on solving problems related to prescribing/dispensing and other areas todevelop the skills of practice and pharmacy staff. Observe theprinciples of clinical governance when developing policies, protocols,guidelines, SOPs and procedures. Person Specification
