Job summary

Readesmoor Medical GroupPractice is looking to recruit an experienced Senior Practice Nurse to join ourfriendly practice team. Salary will beoffered depending on experience. This is a full time position.

At Readesmoor, you will be working alongside an experienced and diverse clinical team to include 4 GP Partners, Salaried GPs, Advanced Nurse Practitioners, Paramedics, Pharmacy Technicians and Practices Nurses with the support of Care Navigators and an administrative team.

Main duties of the job

The post holder is expected to deliver a high level of skilled and planned nursing care to all patients and their families. Supported by the Clinical Lead GP and other Practice Nurses you will work autonomously within your sphere of competency, whilst being a support and resource to less experienced members of staff. You will be able to assess patients, carry out relevant tests and arrange follow-up whilst offering advice and information; demonstrating critical thinking skills in clinical decision making whilst working effectively as part of a multi professional team within a fast pace environment, in line with the protocols and policies and clinical guidelines. Overall the post holder will work collaboratively with the general practice team to meet the needs of the patients, delivery of policies and procedures and providing nurse leadership as required.

Job responsibilities

Act as a positive role model to staff and patients.

Assess individuals health needs using an evidenced basedframework.

Provision of holistic care.

Administer treatment within protocols, policies and clinicalguidelines.

Review medication for therapeutic effectiveness appropriate tothe patients needs in accordance with evidence based practice and clinicalguidelines.

To maintain competencies associated with the role ensuring youare working within your own limitations / scope of practice.

Administer medication within Patient Group Directions or PatientSpecific Direction and to understand these methods of delivering medication.

Demonstrate advanced clinical skills in aspects of chronicdisease management appropriate to your scope of practice.

Demonstrate advanced clinical skills in all aspects in thedelivery of the immunisations and vaccinations service.

Referral / sign posting to appropriate health professionals /services.

Provide patient information and education.

Provide supervision and support of Healthcare Assistants at the practice.

Ensure clinical protocols are adhered to at all times.

Work at all times within the NMC Code of professional practice.

Support line managers.

To participate and assist in the setting of standards of patientcare based on sound research findings.

To complete on-line and face to face training as required, maintainingcompetence and professional development.

Demonstrate flexible and innovative ways of working to provideaccess to the patient population.

Maintain confidentiality and trust at all times.

Adhere to consent and understand the working principlesinvolved, especially in relation to vulnerable groups.

Clear contemporaneous recording and updating patient carerecords.

Maintaining a portfolio of professional development for NMC revalidationto maintain professional registration.

Undertakeclinical reviews and advised on medication changes to patients presenting witha range of conditions which include: Long Term Conditions, Diabetes, Asthma,COPD, CKD, Hypertension, Cerebrovascular disease, Dementia and Coronary HeartDisease in line with practice protocols

WomensHealth/Family Planning including HRTManagement and ring pessary fitting.

CervicalCytology

Vaccinationsincluding Travel Health

LearningDifficulties/Mental Health and Cancer Reviews

Referpatients independently to other clinical disciplines internally and externallyas appropriate for further care

Deliver a comprehensive range of nursingservices if necessary which could include:

v Venepuncture,

vElectrocardiograms,

vEar examination and irrigation (if appropriate),

vTesting and recording patients temperature, pulse, bloodpressure and urinalysis,

vMinor and complex wound management, including referral (whereappropriate)

vNHS health screening,

vPromote and participate in the chlamydia screening programme,

vDiabetic Foot Checks

ImplementNational and Local immunisation programmes for both adults and children underan individualised prescription or Patient Group Directive.

Providenurse cover for treatment room, as required. This will include care forpatients presenting with uncomplicated dressings.

Preventinappropriate admission to hospital with patient education.

Identifyvulnerable patients and set action plans for lifestyle changes.

Accessand Support preparation, management and after care of patients having minorsurgery on the premises.

Undertakeopportunistic health screening of patients in line with practice policy andoffer appropriate advice.

Supportpatients to adopt health promotion strategies that promote patients to live healthilyand encourage self-care.

Providean opportunity for patients to share emotional and psychological issues withina consultation.

Provideappropriate anaphylactic and emergency care according to practice guideline.

Itis necessary to contribute fully in any local or national initiatives,procedures and targets for example, Quality Outcomes Framework (QOF) andenhanced services etc.

Recognisethe signs of child abuse, family violence, vulnerable adults, substance abuse,addictive behaviours, suicidal tendencies and be aware of local, referralprocedures. Assist in Safeguarding Report writing

Undertakeannual training and demonstrate competency in anaphylaxis and resuscitationtechniques.

Carryout home visits (where appropriate) for patients who are not able to attend thepractice for routine nursing procedures where a visit from a member of thecommunity nursing team is not appropriate.

Participatein the administrative and professional responsibilities of the Practice.

Ensureaccurate and contemporaneous notes of all consultations and treatments arerecorded in the patients notes on the clinical computer system as appropriatein accordance with NMC and local policies.

Ensureaccurate completion of all necessary documentation associated with patient healthcare and registration with the Practice.

Ensurecollection and maintenance of statistical information required for regular andad hoc reports.

Attendand contribute in Practice meetings as required.

Assistin the formulation of Practice values and behaviours, strategy and policy anddevelop appropriate protocols.

Ensurethat efficient and appropriate stock control is in place.

Qualifications

Essential

Registered General Nurse Educated to degree level or equivalent 2-3 years experience in general practice Evidence of ongoing personal development Demonstrates an understanding of clinical governance requirements Ability to assess and manage patient risk effectively and safely Ability to work autonomously and as part of a team Resilient and able to work under pressure Ability to deal with challenging situations Evidence of working within multi-disciplinary teams IT literate with well-developed IT and data collection skills Has an understanding of local and national issues influencing the delivery and development of health care Support and Management of Health Care Associates and other junior nursing staff as required

Desirable
