Job overview

The post holder will play a vital role in ensuring efficient, appropriate and safe patient flow from acute hospitals, utilising the bed capacity at St Pancras and following the journey of each patient from admission through to their discharge.

This will be achieved through developing excellent working relationships with acute discharge teams as well as the St Pancras rehabilitation wards, and working with patients, families and key stakeholders to ensure that transfer back to community life is seamless. The post holder will link with the Central Access Team and the Leadership team to provide information, data and support daily reporting locally and across North Central London Partners.

Main duties of the job

To provide high quality admission and discharge planning and co-ordination including administrative and practical support to Clinicians as required

To appropriately prepare and plan patient admissions to the unit, factoring in specialist requirements, ensuring appropriate staffing and infection prevention control measures are in place to retain safety across the unit

To proactively pre-plan admissions and discharges across the unit to maintain patient flow 

To maintain an overview of bed capacity for the unit and report any issues to the SPRU Operational lead or Head of IDT

To ensure high quality communication occurs between St Pancras Rehabilitation and key stakeholders (patients, their families, support services and community teams both statutory and voluntary).

To participate and lead (in absence of patient flow) proactively within the daily board rounds and teleconferences in identifying and addressing the needs of patients and tracking patient flow to discharge. 

To manage out of hours, pre-planning patients ahead of the weekend and communicate updates / issues to the bleepholder

To run required reporting for bed capacity / barriers to discharge / referral management

To act as the crucial communication link between the IDT and SPRU wards in relation to patient discharges

To demonstrate the ability to work independently with supervision and seek appropriate support as necessary.

Detailed job description and main responsibilities

To work collaboratively in identifying barriers and the prioritisation of discharge activities (ward and community) to support positive patient outcomes.

To ensure high quality communication occurs between St Pancras Rehabilitation and key stakeholders (patients, their families, support services and community teams both statutory and voluntary).

To participate and lead (in absence of patient flow) proactively within the daily board rounds and teleconferences in identifying and addressing the needs of patients and tracking patient flow to discharge. 

To act as a specialist resource to support multiborough sign posting activity prior to and on discharge. 

To work collaboratively with the Integrated Discharge Team and the St Pancras Operational and Clinical Leadership Team to avoid delays in patient pathways

To coordinate care and support in a timely way to avoid any system blocks and facilitate discharge

To manage out of hours, pre-planning patients ahead of the weekend and communicate updates / issues to the bleepholder

To run required reporting for bed capacity / barriers to discharge / referral management

To act as the crucial communication link between the IDT and SPRU wards in relation to patient discharges

To review and escalate complex cases where discharging barriers are identified and require additional senior support to enable smooth discharge

To demonstrate the ability to work independently with supervision and seek appropriate support as necessary.

To ensure that tasks/ actions from the Multidisciplinary meetings and board rounds are followed up and actioned in the absence of the patient flow. 

Ensure compliance with Trust and divisional policies and procedures

To consistently work to agreed CNWL, CCG and National Policies, Protocols and Procedures including those related to patient discharge (e.g. ensuring the principles of Discharge to Assess are embedded into practice).

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
