About the Role

We are looking for anexperienced Paramedic Advanced Clinical Practitioner with a digital badge inadvanced clinical practice. You will need to be confident in managing ademanding workload, working at an advanced clinical level and experienced inassessment, diagnosing, planning and treating undifferentiated conditions. Main duties of the job

Our service operates between Monday to Friday, with no late shift, nights, weekend or bankholiday requirement. We are looking to recruit a paramedic ACP to work at MustersMedical Practice, West Bridgford. The range of patient presentations in a GPpractice is vast, from young patients to older persons, mental health, womenshealth, long term conditions, non-acute, acute and chronic presentations. Itsa varied role, thats different every day. You will be working directly withthe GPs, and practice nurses, along with the wider non clinical team, adminand reception staff.

Main duties of the job

You need to be a registeredparamedic, qualified advanced clinical practitioner with digital badge and anon-medical prescriber. This role will require you to be an experienced, clinicallysound and an adaptable clinician, recognising your own strengths andlimitations, to provide clinical assessments, examination, request investigations,diagnose, plan care and treatments and evaluate the care. This role will giveyou opportunity to extend and expand your clinical knowledge and working in aprimary care setting, with a multi-disciplined team in the practice.

The post holder will work within Musters Medical Practice as part of the practice team but will be employed and part of the wider PartnersHealth GP federation team. PartnersHealth employs pharmacists, ACPs, paramedics, physician associates, nursing associates, social prescribers, mental health practitioners, first contact physiotherapists and care co-ordinators and works closely with the 11 GP practices in Rushcliffe. The PartnersHealth additional roles team, which this post is part of, is a growing organisation, that is known for being a forward thinking organisation, continually learning and developing and ultimately delivering high quality care to the population of Rushcliffe.

PURPOSE OF THEROLE

The postholder will hold an ACP digital badge qualification. They will provide care forpatients through advanced clinical assessment, examination, investigation,diagnosis, care planning and treatment, evaluation and follow up of care in aclinic room setting or in a patients own home as a domiciliary home visit.

Post holderwill deliver a high standard of patient care with in-depth theoreticalknowledge and evidence-based practice, working autonomously, and as required withsupport and supervision of a GP.

The postholderwill deal with presenting patients needs in a primary care setting.

The postholderwill work within their clinical competencies, agreed with the GP supervisor.

MAIN DUTIESAND RESPONSIBILITIES

The postholder will provide first point of contact care for patients presenting withundifferentiated, long-term conditions and undiagnosed problems utilisinghistory taking, physical examinations, problem solving and clinical decisionmaking skills to establish a working diagnosis and management plan inpartnership with the patients (and their carers where applicable)

Instigatenecessary invasive and non-invasive diagnostic tests or investigations andinterpret findings/reports within your scope of competence. Discuss the resultand implications of laboratory investigations with patients.

As anindependent prescriber, you will utilise local and national clinical guidelinesto prescribe medications as appropriate and promote evidence-based practice.

Deliverintegrated patient centred care through appropriate working with the widerprimary care network multi-disciplinary team and social care networks

Undertake faceto face, telephone, video and online consultations for emergency and routineproblems, as determined by the practice and the PartnersHealth, includingmanagement of patients with long term conditions.

Where requiredand within skill competence and relevant training, the PCN paramedic ACP canoffer specialised clinics, including but not limited to family planning, babychecks, COPD, asthma, diabetes and anti-coagulation. This is not an exhaustivelist.

Supportthe nursing staff and Primary Care Team as required.

Undertake homevisits and participate in duty rotas as agreed with the practice managementteam.

Providehealth/disease promotion and prevention advice to patients

Utiliseclinical guidelines and promote evidence-based practice, participate inclinical audits, significant event reviews and other research and analysistasks.

Utiliseavailable diagnostics, plus interpretation, to support diagnosis, treatment,and management plans.

The postholder will ideally be a non-medical prescriber. If not already a non-medical prescriber, youwill be supported to complete your non-medical prescribed as part of the ACPpathway.

The postholderwill work to the ACP roadmap for continuous and ongoing CPD.

ADMINISTRATION

Ensure continuing education,training and development is undertaken to meet clinical governance guidelinesfor Continuing Professional Development and a Personal Development Plan.

Maintain and evidence all competencies toretain the digital badge status.

Maintain professional registration with theHCPC as required to maintain the healthcare professional registration.

Undertake a fair share of the overalladministration associated with clinical practice in primary care.

Involvement in clinical audit as part of newservice implementation, quality improvement, patient reviews, QOF and any otheraudit work undertaken by the practice and/or PartnersHealth

Show a caring attitude towards patients and show acaring attitude towards patients, have well-developed communication skills toprovide treatment plans. And work to a professional code of ethics

Keep up to date with relevant medical research, technology, and evidence-basedmedical practice by attending continuingeducation courses and professional meetings, reading journals etc.

Experience of working as a paramedic, with advanced clinical skills and a digital badge qualification, including non-medical prescribing. Using advanced clinical skills, undertake clinical assessments, including comprehensive history taking, examination, initiate investigations, diagnose, plan care, and prescribe medications as required Clinically competent and experienced managing various undifferentiated conditions, from acute to chronic, non-acute/non urgent, urgent presentations, mental health, long term conditions, young patients/children to older and frail patients, the list is not exhaustive.
