Main duties of the job

To work as part of a multi-disciplinary team to ensure high quality and consistent patient outcomes and experience for our registered patients.

Our clinical team is supported by salaried GP's, and Advanced Nurse Practitioner, 3 Clinical Pharmacists, 2 Practice Nurses and 3 Health Care Assistants.

We also have a large non-clinical administrative team.

Job responsibilities

Chronic disease management, ensuring care plans are updated and regularly reviewed. Diabetes COPD/Asthma Cervical Cytology Requesting pathology tests for urine culture, swabs. Wound care and treatment of minor injuries. Removal of sutures. DES/QoF related clinical and administrative work. Routine immunisations/childhood immunisations. Influenza, Pneumococcal, shingles and pertussis in pregnancy vaccinations. Ordering of vaccinations, maintaining stock levels, ensuring stock rotation and cold chain policy is adhered to at all times. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures. Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. Act as an advocate when representing patients and colleagues. Participate in practice team meetings. Utilise the audit cycle as a means of evaluating the quality of work of self and the team, implementing improvements where required. Participate in shared learning across the practice and wider organisation and assess the impact of policy implementation on care delivery. Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model. Qualifications

Essential

Registered adult nurse on the NMC register without restrictions. GCSE Grade A-C (or equivalent) in English and Maths.

Desirable

Qualification in Asthma/COPD. Non-medical prescribing qualification. Accredited long term condition modules such as Hypertension, Coronary Heart Disease, Chronic Kidney disease. Cervical cytology sample takers registration. Diploma in Diabetes.

Experience

Essential

Prior work as a Diabetes practice nurse. Experience working within a nursing team. Knowledge and understanding of general responsibilities to patient information and record keeping.

Desirable

Experience of working in a GP Practice. At least 1 year post qualification experience.

Additional

Essential

Excellent interpersonal and communication skills. Car driver with a clean license. Flexible individual who can adapt to business needs, able to cover for colleague absence through sickness, annual leave or study leave. Computer literate.
