The post holder will:

Assist the ICB in delivering its commissioning responsibilities as required in The National Framework for Continuing Healthcare and Funded Nursing Care.

Undertake clinical assessments and reviews of Continuing Healthcare eligibility for adults of all ages utilising the National Framework for NHS Continuing Healthcare and Funded Nursing care.

Work with professionals, providers, individuals and their families and/or representatives to make determinations regarding nursing care need requirements for new and existing service users.

Support in monitoring the standards of care delivered by care providers, giving advice and guidance to care staff regarding individual service users care needs in line with clinical governance.

Operational

Has an understanding of other agencies policies and procedures and to work in accordance with relevant legislation and acts, including The Community Care Act and Mental Health Act.

To have knowledge of record keeping systems for audit and resource development at local level

Main duties of the job

Communication

Assessfor and communicate sensitive and complex medical/nursing information/needseffectively to patients and carers. Evenwhere there may be barriers to understanding such as mental healthproblems.

Communicates sensitive and complex medical/nursing information effectively, accurately, and concisely to colleagues and professionals of other agencies.

Has a knowledge of the pathway and signpost people with verbal complaints - including other professionals, patients, carers, and relatives.

Education, Teaching and Training

Educates, with regard to Continuing Care, nursing and social care staff of other agencies within both the private and public sectors.

Participates in training and induction of new staff members to Continuing Care, evaluating and feeding back information.

To undertake training as identified appropriate to the role and identified through the appraisal process.

To maintain the confidential nature of clinical and management information within the requirements of the Data Protection Act, Caldicott guidelines and Trust Policies with regard to confidentiality.

Operational

Has an understanding of other agencies policies and procedures and to work in accordance with relevant legislation and acts, including The Community Care Act and Mental Health Act.

To have knowledge of record keeping systems for audit and resource development at local level.

Developing and Maintaining Relationships

Actively works towards developing and maintaining effective working relationships both within and out of the CCG.

Fosters and maintains strong links with all Services across the CCG, developing key contact points within Care and Corporate services.

Develops and maintains good relationships with patients, carers, colleagues and professionals from other agencies in both the public and private sectors.

Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.

Person Specification

Qualifications

Essential

Registered Nurse (part 1 of the Nursing and Midwifery Council (NMC) register as a registered Nurse Educated to degree level or relevant clinical training and experience to degree equivalent level in related function Experience of direct patient care involving people with complex health needs.

Experience

Essential

Evidence of up-to-date clinical practice Evidence of on-going professional development Experience of working to and achieving tight deadlines Evidence of patient facing experience and ability to manage challenging situations Knowledge of working with persons with physical disabilities, learning disabilities and/or adverse mental health experiences.

Desirable

Knowledge of the National Framework for Continuing Healthcare and the Mental Capacity Act and associated guidance/policies. Hospital or Community nursing experience
