Job summary
Role
To work as a full member of the primary health care general practitioner team providing a full range of personal medical services to patients and contribute to the development of the practice.
The Core activities of the General Medical Practitioner:
Provide a full range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate. Provide other personal medical services to meet identified patients need. Make professional, autonomous decisions in relation to presenting problems whether self-referred, or referred from other healthcare works within the organisation. Assess the healthcare needs of patients with undifferentiated and undiagnosed problems. Screen patients for disease risk factors and early signs of illness. Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education. Work within current CCG policy for prescribing.
Main duties of the job
Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard. Compile and issue computer generated acute and repeat prescriptions (avoiding hand written prescriptions whenever possible. To take part in areas including health research, clinical auditing and data collection. Contribute to QOF requirements and recording data appropriately. Screening the patients for health education. Attend team meetings and other clinical meetings as necessary. In general the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working with primary care, as set by Royal College of General Practitioners, General Medical Council, all other Royal Colleges and Councils applicable to the staff within the Practice, Department of Health, CCG, CQC, and all other regulatory and standard setting organisations.

Be involved in developing primary care servicesto meet the needs of the practice population. Working in close collaboration with the practice team to ensure that access targets are reached. Work with the practice team to ensure that immunisation and health inequalities targets as defined within the PMS contract are reached. Support health development and ensure health promotion work is undertaken to allow the practice population to make informed choices. Ensure the practice is represented at appropriate meetings, eg Practice Based Commissioning Groups.Managerial

Prescribe safe and effective medicines management which conforms to: NICE Guidance and DH directives Good Prescribing Practice as defined by British National Formulary (BNF) guidance from Royal Pharmaceutical Society of Great Britain on safe and secure handling of medicine Operate safe management and regular review for all patients receiving repeat prescriptions Controlled Drugs Legislation Shared protocols agreed between the CCG and other NHS providers Locally specified Patient Group Directions Work effectively so as not to exceed the Practice prescribing budget as set annually by the CCG. Comply with targets set within the Prescribing Incentive Scheme. Contribute to the teams approach which ensures that referrals are made for patients to see other medical service providers, ensuring that adequate information is given to personnel to whom the patient has been referred for proper assessment to be made. Be supportive of any complaints policy and procedure to ensure proper investigation and appropriate follow-up action in the event of a complaining being made about any aspect of the service provided or any personal provider of the service.Confidentiality
All patients have the right to expect that all practice staff will respect their privacy and act appropriate at all times. In the performance of duties the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as business organisations. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may be divulged to authorised persons in accordance with the organisations policies and procedures relating to confidentiality and the protection of personal and sensitive data.Communication
