Reablement Worker

The Care4CE Community Reablement Team within Cheshire East Council is recruiting for frontline Reablement Workers across all areas of Cheshire East.

The head office is at Macclesfield Town Hall in Macclesfield with a satellite office at Westfields in Sandbach and the teams are split north and south.

The service contracts are permanent working 7am-11pm although our main core hours are 8am-10pm. Various shifts and contracts on offer which include travel time from 15 hours up to 37 hours per week.

Grade.5 start pay of £12.59 per hour incrementally over 4 years increasing to £13.47 per hour with a yearly pay rise.

Alternate weekend working is paid at time and half as are bank holidays except Xmas Day and Boxing Day are double pay. Shifts comprise of a mixture of mornings, afternoons, and evenings dependent upon contract hours.

You need to be a car driver with mileage paid at 52.2p per mile over 1,000cc. Electric and under 999cc engines are paid at 46.9p per mile.

Business use is required on your car insurance. A Uniform and DBS are provided.

ABOUT THE ROLE

Reablement is part of the Integration Home First Agenda between Social Care and the NHS and is a short-term intervention to encourage individuals to achieve maximum independence with personal care and daily living skills, working in a person-centred manner. Our remit is to support discharges from the hospitals to complete assessments to Reable individuals back to independence or for ongoing care needs and rehabilitation of therapy exercises.

We also support hospital avoidance, working alongside the Urgent Community Response/Virtual Ward teams providing a same day response over a 72hour period when someone has a health crisis and are supported at home rather than being admitted to hospital when safe to do so, this also includes low level health care tasks, such as taking blood pressure, temperature, pulse, emergency first line simple wound dressings and end of life.

Full training and induction is provided.
