Main duties of the job
We are looking for a Band 5 Wound Care Community Nurse to join our Community Nursing Team based in the Herne Bay area. You will provide high quality, specialist care to housebound patients with complex wounds in patient's homes and in a clinic setting.
You will be responsible for the running of the day-to-day Wound Clinics and ensure close collaborative working relationships with Primary Care Networks, Community Nursing Teams and other members of the MDT, providing shared care to meet the health and social care needs of the local population.
The Wound Management Centre is offered to housebound adults with patient transport and 5 days a week including bank holidays. The shift patterns cover 8am until 5pm.
