Job summary

This post is to work at a clinical and leadership level to coordinate delivery of patient care and deliver on KPIs, within the aims of the role and wider Epilepsy Specialist Nurse Pilot and is supported by Buckinghamshire Oxford and Berkshire West Integrated Care Board (BOB ICB) on behalf of the NHS England.

You will ideally be looking to particularly work with, and improve outcomes for, those from areas of higher deprivation or with additional needs such as medical complexity, learning disability and autism, or transitioning to adult care.

The successful candidate will be expected to work as an integrated member of the Clinical Epilepsy Team and demonstrate advanced clinical skills and deliver the KPIs, within the aims of the role and wider pilot.

The role will be supported by the Band 8 Lead Epilepsy Specialist Nurse who will provide clinical leadership and support across the nursing team, to ensure the delivery of safe and effective care to children and young people with epilepsy within the BOB ICB System.

We are looking for individuals who are experienced in caring for children and young people with epilepsy, and who possess the skills required to work proactively and take the initiative. The role will involve working with a number of different teams to implement the pilot within each provider and will feed into the wider epilepsy teams within BOB ICB, providing an excellent opportunity to extend and develop existing skills in caring for children and young people.

Main duties of the job

The post holder will support the Epilepsy Clinical Lead and their team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery. You will be managing a complex caseload of CYP with epilepsy who require co-ordination of care across multiple settings and services.

Job responsibilities

Dependent on experience and discussion with the team, leading on a specialist area of epilepsy in line with the priorities of this pilot (such as transition, medical complexity or learning disability and autism).

Undertaking wider system-level roles to support horizontal and vertical integrated care:

o Ensuring, and establishing where required, relevant referral pathways into services such as mental health care, tertiary neurology and neurodevelopmental services.

o Supporting transition of YP into adult epilepsy services, specifically YP with complex epilepsy requiring a multidisciplinary team approach.

o Liaising with community paediatric services in the management of CYP with learning disability and autism (LD&A). For example, this may include establishing and supporting any out-reach clinics in residential school settings.

o Developing a sustainable model for training schools in the use of emergency medications

o Support consenting processes required for genomic investigations.

Where appropriate, and ensuring that the model used within the provider remains focussed on the aims and purpose of the Pilot, the role may be used to release time from the existing Epilepsy Nurse Specialists, in order to allow them to focus on specific areas of Service Development
