Job summary

This is an exciting opportunity for a dynamic, person centred, Specialist Physiotherapist, with great leadership and interpersonal skills. You will be leading a high quality Physiotherapy service to adults with learning disabilities who are referred to the Shropshire and South Staffordshire Community Learning Disabilities Teams (CLDT) and Intensive Health Outreach will lead a highly specialist physiotherapy service to adults with complex learning and physical disabilities, undertaking clinical diagnoses, assessment and physiotherapeutic management and interventions.Deliver a high standard of care using a holistic evidence-based person centred approach on a daily basis. The role will be split equally between clinical practice and providing clinical leadership, teaching and supervision to members of the physiotherapy and multi-disciplinary members within the Adult Learning Disability Service across Shropshire and South will be an autonomous practitioner, holding a small mixed clinical caseload of those with mobility, postural management and respiratory needs. Working without direct supervision within the context of a multi-disciplinary team.

Main duties of the job

To lead a highly specialist physiotherapy service to adults with, learning and physical disabilities and occasionally to adults with complex developmental disorders who are unable to access mainstream services with reasonable adjustments. Undertaking clinical diagnoses, assessment and evidence based physiotherapeutic treatment programmes, management, and interventions. To deliver a high standard of care using a holistic evidence-based person centred approach on a daily basis. The role will be split between clinical practice and providing clinical leadership, teaching and supervision to members of the physiotherapy and multi-disciplinary members within the Adult Learning Disability Service across South be an autonomous practitioner, holding a small mixed clinical caseload of those with mobility, postural management and respiratory needs. To support less experienced staff and students and colleagues deliver the Trust contracted provide expert advice in relation to Physiotherapy in adults with learning disability; signposting and providing guidance and information to health and social care professionals, clients, carers and relatives.

Job responsibilities

Be professionally accountable for all aspects of own work, including caseload management and direct clinical care. To work within the Chartered Society of Physiotherapists and Health and Care Professionals Councils code of conduct and scope of practise.

Obtain and record informed consent from clients at each contact in accordance with Trust policy.

Carry out comprehensive specialist Physiotherapy assessments for clients with complex learning and physical health needs. Interpret clinical information and data related to clinical parameters along with non-clinical information and use clinical reasoning skills to accurately diagnose and develop specific treatment plans.

Provide specialist advice to other professionals or single assessments to meet clients holistic care needs.

Formulate and deliver individual treatment plans based on specialist knowledge, evidence-based practise, national and local guidelines using profession-specific clinical skills and techniques. ( 1:1 interventions, carer education, patient education).

Maintain and share up to date knowledge of Physiotherapy assessment and intervention evidence base developments for this client group.

Maintain and share up to date knowledge of instrumental assessment available and to be able to signpost to services that assess and provide this.

To ensure physiotherapy representation at the weekly CLDT meetings and contribute to the multi-disciplinary clinical referrals, assessment and discharge planning process and onward referral as required.

Prioritise urgent or unexpected or planned referrals, adjusting caseloads and allocations to meet revised needs.

Undertake other duties when requested by professional or operational managers.

Maintain a professional portfolio to demonstrate competency to practice and which meets the requirements of professional registration.

Demonstrate the ability to facilitate team working across organisational boundaries and with healthcare professionals outside of the Trust.

Be a responsible, clinically autonomous practitioner, working within appropriate professional guidelines and code of conduct.

Triage, prioritise and manage Physiotherapy Adult Learning Disability clinical caseload.

Assess and plan therapeutic interventions from initial referral to discharge, and onward management which may include very long term plans and contribute to care packages for clients with chronic, complex conditions.

Person Specification

Qualifications & Training

Essential

Diploma or BSc in Physiotherapy. Masters level modules or working towards in relevant clinical area, research or Leadership. Unrestricted registration with Health and Care Professions Council (HCPC). Extensive post registration courses and experience.

Desirable

Post graduate research qualification learning and publication in subjects such as; o Bobath techniques o Rebound Therapy o Postural management o Hydrotherapy.

Experience

Essential

Extensive post graduate experience including rotational across a range of clinical settings, including Learning Disability. Competence with clients with complex learning and physical health needs including hydrotherapy, postural management and respiratory care. Experience as a senior practitioner in a relevant area of physiotherapy assessing, diagnosing and treating a wide variety of complex presentations and disorders.

Skills, Knowledge & Abilities

Essential

Demonstrated knowledge and skills and competencies associated with working across a wide range of patient pathways relevant to and including people with learning disabilities and complex health needs.
